Description

PERFORMANCE BASED STATEMENT OF WORK Steriliz Rapid Disinfector Uv Disinfection 1. GENERAL: The contractor shall provide all labor, supplies, tools, materials, transportation, parts and equipment necessary to support four (4) Steriliz Rapid Disinfector UV Disinfection units located at the William Jennings Bryan (WJB) Dorn, Veterans Affairs Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209. 2. BACKGROUND: The WJB Dorn Medical Center is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 83,000 patients. 3. PERIOD OF PERFORMANCE: One Base year & four one-year option periods. 4. PLACE OF PERFORMANCE: William Jennings Bryan (WJB) Dorn, Veterans Affairs Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209. 5. SERVICES TO BE PROVIDED: The Vendor will extend all of the terms, conditions, responsibilities and limitations, pursuant to the original Warranty to include the following: Product training and Q&A upon request: onsite 1 day/year (up to 6 hours per day) and /or online webcast 4 times per year. Software updates for software on the R-D machine usually 2 to 4 times per year, as needed 4G network access coverage Online data access and reporting includes operation, utilization, trigger emails, utilization emails, account review to help drive system utilization Routine inspections as needed Repairs and maintenance includes all parts and labor for all system component repairs (lamps, ballasts, computers, sensors, batteries, etc.). Plug ends are not included. Plug end replacements are available but customer replacement of plug end will NOT void warranty. Sensor recalibration 1 time per year (4 UVC Sensors) to be completed upon service agreement renewal or as needed.
