- Attachment D - Statement of Work Attachment C - Tax Exemption Attachment B - BAA Attachment A - Rate Schedule RFP 18-247-SOL-00046 Requirement is set aside for Total Small Business. Contractor shall provide non-personal Pediatric Dental Services at the Hopi Health Care Center, located in Polacca, AZ on an as needed basis in accordance with the terms specified in the attached statement of work and contract terms and conditions. Period of Performance is estimated to start on October 1, 2018 through September 30, 2019. This requirement is being solicited for a base and four (1 year) option years. Contractor shall state in their proposal that the quote shall remain effective for 120 calendar days after close of solicitation. Closing Date is August 31, 2018 at 1700 hours Arizona time. Deadline for questions is on August 28, 2018, 1700 hours Arizona time. The Government reserves to right to make a single award, multiple awards or no awards in accordance with provision at FAR 52.215-1 which is included in this solicitation. Contractor is to follow and submit proposals in accordance to Section E Solicitation Provisions and Addendum to FAR 52.215-1 Instructions to Offerors - Competitive Acquisitions.
