Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: 36C25718Q9811 Solicitation Type: Request for Quotation (RFQ) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 eff. 07-15-18. This acquisition is 100% set-aside for Small Businesses. The North American Industry Classification System (NAICS) Code associated with this procurement is 333318 - Food warming equipment, commercial-type, manufacturing, and the size standard is 1,000 employees. Tiered evaluations shall be used. See attached Notice of Tiered Evaluations for details. See attached Statement of Work Cook-Serve Dining Kiosks for a description of the requirement. See attached Floor Plans for drawings of the various sites. See attached Listing of Items Required for Contract Line Item Numbers (CLINS), items, quantities and units of measure. The Period of Performance shall be no later than 180 days after award. The principal Places of Performance are as follows: Audie L. Murphy VA Medical Center 7400 Merton Minter Blvd. San Antonio, TX 78229 Kerrville VA Medical Center 3600 Memorial Blvd Kerrville, TX 78028 Polytrauma Transitional Rehabilitation Program Center 4949 Gus Eckert Road San Antonio, TX 78240 Site visits will be available on August 23, 2018. Offerors requesting a site visit must submit an email to andrew.kinh@va.gov with the below information no later than August 21, 2018: 1. Full name of each person visiting site 2. Email address of each person visiting site 3. Company name 4. Company DUNS Number The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at FAR 52.212-2, Evaluation Commercial Items does not apply to this acquisition. Award shall be made in accordance with FAR 13.106-2(b) to the offeror whose quote offers the best value to the government, considering technical capability and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability that meets or exceeds the requirement, and (2) price. Technical acceptability is defined as conformance of the quote to the Statement of Work. All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items with their quote. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. Addenda are attached to the clause.
