- Related Experience Form NAVFAC/USACE PAST PERFORMANCE QUESTIONNAIRE (Form PPQ- Single Source Determination - HST Studs Bolts-Single Source Determination W912DQ18R1045 TRUMAN SHAFTS The U. S. Army Corps of Engineers, Truman Power Plant, 29564 Powerhouse Road, Warsaw, MO 65355, has a requirement for a supply contract for the fabrication of stub shaft studs, turbine main shaft studs, and the Mulit-Jack Bolt Tensioners and associated hardware. The required fasteners are manufactured by Nord-Lock, - Superbolt, Inc., 1000 Gregg St., Carnegie, PA 15106, USA. The contract includes the procurement of 80 Superbolt MT- 03013 supernuts, and 55 Superbolt MT- 02986 supernuts and all associated parts including nuts, washers, jackbolts, O-rings, and jackbolt plugs as well as packaging and shipping, and the fabrication of machined type main shaft studs and rolled type stub shaft studs, including all costs associated with labor, equipment, materials, testing, packaging and shipping.
