SWL Hoist & Hangar Bar Room Covering Track System: 4m x 4m (H track) Basic Seated Sling, Associated Hardware LOCAL STOCK NUMBER: GH3 600LBS 16.00 EA $00.00 $00.00 0003 Certified Installation of Track and Structure Supports (Above Ceiling Connection) *Owner responsible for providing 110v Power Connection LOCAL STOCK NUMBER: LABOR 17.00 EA $00.00 $00.00 GRAND TOTAL $00.00 SALIENT CHARACTERISTICS FOR PATIENT LIFTS IN THE INTENSEIVE CARE UNIT (ICU/2J) AT CENTRAL TEXAS VETERANS HEALTH CARE SYSTEM The ICU ceiling mounted patient lifts must have the following minimum characteristics in order to meet the required specifications for this project: 1. 16 patient lifts must guaranty a 600 lbs. lift capacity and one patient lift must guaranty an 825 lbs. weight capacity. 2. Contractor must show certification for safety compliance for the ceiling lift and charging system. 3. Each overhead ceiling mounted patient lift system must be complete with a track system, lift unit, motor, and complete structural mounting system to attach to the existing structure. 4. Each ceiling track must be made from high strength extruded aluminum at a thickness of at least 3/16. 5. Must include anchor supports at a minimum of 3 per linear foot on the ceiling surface 6. The lift unit itself must be made of a steel frame system, driven by a gear reduced high torque motor that includes the following characteristics: - Lifting capacity: 16 at 600 lbs., 1 at 825 lbs. - Electronic soft-start and soft-stop motor control - Emergency lowering device - Emergency stopping device - Current limiter for circuit protection in case of overload. - Safety device that stops the motor to lift when batteries are low. - Lifting speed: 2.2in/s (6 cm/s) in full capacity - Emergency brake in case of mechanical failure - Strap length up to 90in (2.3m) tested for 2998lbs (1360kg) - Integrated scale and lift quantity calculator 7. The motors for the patient lift system must include the following characteristics: - Vertical movement motor - Horizontal movement motor - Fully enclosed motor system - Secured to the chassis 8. Must include rechargeable batteries that will provide for a minimum of 120 patient transfers at a load of 200 lbs. or 70 patient transfers at a load of 660 lbs. 9. The charger must provide continuous charge via lined rail system with input at 100-240 volts and output at 27 volts 10. Each lift must include a patient sling with lifting capacity of 600 lbs. For 16 lifts and 825 lbs. for one lift 11. Installation must be completed under the supervision of a qualified representative of the contractor to manufacturer s specifications 12. Contractor must provide in-service training to ICU staff 13. Contractor will demonstrate to the Central Texas Veterans Health Care System Engineering Department that the equipment operates properly in accordance with design and specifications at the end of installation 14. Contractor must provide a 3-year parts and labor warranty, to include batteries STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: Central Texas Veterans Health Care System (CTVHCS) Intensive Care Unit Ceiling Patient Lift System purchase and installation. 2. Scope of Work: The contractor will uninstall the current 17 ceiling suspended patient lifts in the ICU and leave them for CTVHCS staff to disposition. The contractor will also provide 16 new ceiling suspended patient lift systems with 600 lbs. capacity and one ceiling suspended patient lift system with 825 lbs. capacity. The contractor will install the 17 new patient lifts in each of the patient rooms of the CTVHCS ICU. 3. Background: The current fleet of 17 ceiling suspended patient lifts in the CTVHCS ICU are in need of constant repair and are no longer meeting the needs of patients and staff in the CTVHCS ICU. As such, CTVHCS is procuring 17 new patient lifts and having them installed at the earliest date possible. 4. Performance Period: Delivery of new patient lift equipment, uninstallation of old patient lift equipment, and installation of new patient lift systems is requested to be completed not later than 9/30/18. 5. Type of Contract: This is a Firm-Fixed Price contract. B. GENERAL REQUIREMENTS Uninstallation of the current patient lift systems will take place on the 2nd floor of Bldg. 204 at the Temple VA Campus at 1901 South 1st St., Temple TX. In addition, Contractor will provide all 17 new suspended ceiling patient lifts equipment and install ceiling suspended patient lift systems compatible with the requirements on the Salient Characteristics document. This work will be conducted in all 17 of the patient rooms in the ICU (Wing 2J). The exact rooms involved are Rooms 2J01, 2J02, 2J03, 2J04, 2J05, 2J06, 2J07, 2J08, 2J09, 2J10, 2J11, 2J12, 2J13, 2J14, 2J15, 2J16, and 2J17, which are immediately adjacent to each other in the CTVHCS ICU. Enclosed is a floor plan of the area. Please note all ICU rooms are not identical in terms of floor plan, but are indeed very similar. ICU patient rooms are marked accordingly in green on the floor plan. CTVHCS welcomes any potential Contractor to visit the site if they wish to. Visitation would take place in accordance with CTVHCS policy regarding vendor/contractor visits. 2. The new ceiling suspended patient lift equipment can be shipped to CTVHCS, 1901 South 1st St., Warehouse 44, Temple, TX 76504, ahead of installation or can be brought on site with the installation crew at the time of installation. Either way is acceptable for CTVHCS. C. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. D. NETWORK AND SENSITIVE INFORMATION The items being purchased will not be connected to the Internet/Intranet nor will it store VA Sensitive information. The contractor will be on site to install items but will not need to access the Internet/Intranet or VA Sensitive information..
