If you do not have file size issues you can download this entire package - 18-235-SOL-00013 Complete Document 7.3 Directory of Health Programs 7.2 Reports, Data and Other Deliverables 7.1 Performance Requirements Summary IHS Business Associate Agrement Contract Clauses performance Work Statement SF-18 (i) This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 18-235-SOL-00013is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses. Quote/Proposal packages must be submitted to the Contracting Officer via email Cordell.Bailey@ihs.gov and in the Subject Line insert, Response to Solicitation 17-235-SOL-00021. (iv) This acquisition is a total small business set aside. (v) There is only one contract line item which is to provide Diabetes Education Professional Consultant services in accordance to the Performance Work Statement (PWS). (vi) The contractor shall provide all services required in the PWS. (vii) The period of performance is one year and will be indicated on the award document. (viii) The attached Clauses and Provisions apply to this acquisition. (ix) The attached Business Associate Agreement will be made part of the contract and the blanks will be filled in by the Contracting Officer with the information of the successful Contractor. (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications­ Commercial Items, with its offer. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal.
