- PWS youth parents family This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is a Total Small Business Set-Aside. The Navajo Area Indian Health Service, Pinon Health Center Counseling Services (PHCCS) is requesting for proposals from interested Offerors to provide consultant contracting services to provide Eight (8) Educational and Experiential Therapy Sessions, Experiential Learning for Youth and Family Projects. Review the enclosed instructions and provide this office with a proposal, utilizing the information contained herein. The Government contemplates award of a firm fixed price contract resulting from this solicitation. Pinon Health Center (PHC) provides medical care for approximately 12,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care. The Pinon Health Center is an Ambulatory Care outpatient facility. Services offered are: Outpatient Primary Care, Podiatry, Women's Health, Mental Health, Pharmacy, Optometry, Dental, PT, OT, Speech Pathology, Audiology, Lab, Public Health and School Health. EVALUATION CRITERIA PHC reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Past Performance, knowledge of Navajo Traditional customs. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. Questions may be submitted in writing via email prior to RFQ Closing Date. To be considered for award, your offer shall include the following documents/information: 1. Provide Price Quotation. The Government will consider competitive rates upon negotiation. 2. Provide evidence in experience in providing services similar to scope and complexity to Federal Government or other large commercial/government entities within the last three (3) years of operation. 3. Provide evidence that your offer meets the attached Performance Work Statement.
