SECTION 1 - SUPPLIES OR SERVICES AND PRICES/COSTS 1.1 BRIEF DESCRIPTION OF SERVICES The Department of Health and Human Services (DHHS), Office of the Assistant Secretary for Health (OASH), The Office on Women's Health (OWH), is issuing this RFQ to create Journalist Fellowship for the Office on Women's Health (OWH). 1.2 TYPE OF CONTRACT The Government has determined to award as a Firm Fixed Price. 1.3 NON-SEVERABLE SERVICES The services specified in this contract (or line item) have been determined to be non-severable services - a specific undertaking or entire job with a defined end product of value to the Government. 1.4 CONSIDERATION AND PAYMENT SCHEDULE The total fixed price of this contract/order is tiny_mce_marker____________. Payment shall be made upon the delivery and acceptance of the services listed in Section 5.3. SECTION 2- DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK 2.1 TITLE Office on Women's Health Journalism Fellowship 2.2 BACKGROUND The Office on Women's Health (OWH) was established in 1991 within the Office of the Assistant Secretary for Health at the U.S. Department of Health and Human Services (HHS). As the focal point for women's health within HHS, OWH coordinates the efforts of all the HHS agencies and offices involved in women's health. Its vision is to ensure that "All women and girls achieve the best possible health." Its mission is to "provide national leadership and coordination to improve the health of women and girls through policy, education and model programs." OWH works to redress inequities in research, health care services, and education that historically placed the health of women at risk. OWH promotes women's health through its 4 basic functions. 1. Coordinating and Promoting Collaborations among HHS Agencies and Offices: OWH serves as the coordinating office for women's health initiatives across the agencies of HHS, including the National Institutes of Health (NIH), the Food and Drug Administration (FDA), the Centers for Disease Control and Prevention (CDC), and other agencies and offices. 2. Providing Information on Research, Prevention and Services to Assist Decision-Makers: OWH convenes and consults with federal departments, the scientific community, professional organizations, and consumer groups to support and advance women's health. 3. Identifying, Developing and Supporting Model Programs and Innovations in Women's Health: OWH promotes the development and implementation of evidence-based initiatives in academic centers and communities to address the health needs of women across different ages, cultures, races, and ethnic groups, including delivery of care to women and education of health care practitioners about women's health. 4. Educating Health and Wellness Professionals and Consumers: OWH promotes comprehensive health services for women across their lifespan. OWH also supports the development and use of culturally appropriate practices in medical education and research, so all individuals and communities benefit. Through its efforts, OWH brings reliable information on hundreds of health topics to women in every region of America. It also helps consumers, associations, and organizations voice their ideas about the federal government's policies on women's health. The OWH Health Journalism Fellowship program was established to: • Increase Visibility for OWH. This fellowship will raise OWH's visibility among health news reporters, writers, editors, and producers as a source of interesting, reliable, and valuable women's health information. Participants are exposed some of OWH's key resources and services, and return to their newsrooms to share this information with their colleagues and fellow journalists. • Improve Knowledge Dissemination. This fellowship will help more journalists know about the information OWH provides thus opening new dissemination channels to the public. This helps OWH enhance its ability to distribute and promote its health education materials more efficiently. Health and medical journalists also enhance their ability to incorporate more of the Office's data, programs, and resources into their news reporting and editing. • Enhance Medical and Scientific Reporting. Through the fellowship, OWH will impact future coverage of health and medicine by helping fellows better understand and obtain evidence-based information on women's health. Reporters will learn about the office's priorities and major projects, how to access women's health information, how to analyze its importance, and then describe its application. The fellows also are exposed to the OWH and other federal women's health subject matter experts to enable them to identify credible sources to cover women's health issues. 2.3 PURPOSE The OWH seeks to establish a journalism fellowship program in order to: raise OWH's visibility and awareness among the news media; help promote and foster the dissemination of women's health information; increase awareness of OWH's resources for health reporting; and enhance the quality and accuracy of today's coverage of women's health issues. The OWH Health Journalism Fellowship is envisioned for at least 5 journalists (screened and selected by an independent journalism association) to participate in a three-day, on-site visit with researchers and senior officials at the Department of Health and Human Services' Office on Women's Health (OWH). Although fellows are sought from each of the three major media traditional and digital media platforms the most qualified applicants who are most likely to make best use of the program should be chosen. The Fellows will come to OWH as a group. Working with an independent, professional development-oriented journalism organization is crucial to the program's success. It creates instant credibility for a journalistic educational development activity and ensures participatory interest among its intended, target audience. The contractor shall demonstrate how it will plan, implement, integrate, and successfully provide each described service or task within the specified period of performance and according to the prescribed timeframe for deliverables. 2.4 STATEMENT OF WORK The contractor shall provide all labor, material, equipment and resources to achieve the Objectives as detailed in Specific Tasks and Deliverables below. The contractor shall be directly responsible for ensuring the accuracy, timeliness and completion of all tasks under this effort. Specifically, the Contractor shall be responsible for the following Tasks: Task 1: Program Management a. Within 7 days of award, meet with OWH to outline goals and schedules. This meeting shall initiate the communication process by introducing key task participants and explaining their roles, reviewing communication ground rules, and ensuring a common understanding of subtask requirements and objectives. b. Conduct regular meetings with OWH to discuss and plan ongoing activities c. Within 3 days of each meeting provide OWH with notes summarizing the discussion, and outlining next steps to be taken Task 2: Fellowship program development a. Work with OWH to develop objectives for program b. Work with OWH to determine criteria for selection of 5 attendees c. Create application form and process for selecting attendees d. Create agenda and run-of-show for program e. Create promotion plan for Fellowship program Task 3: Fellowship program implementation a. Promote program to members b. Select attendees using form and process created c. Notify attendees of selection d. Arrange travel for selected attendees e. Manage logistics for program f. Summarize program for members Task 4: Fellowship program evaluation a. Work with OWH to determine evaluation criteria b. Develop an evaluation instrument c. Data collection for evaluation at completion of program d. Draft evaluation report e. Finalize evaluation report Task 5: National Meeting planning and implementation a. Work with OWH to determine agenda for women's health segment of meeting b. Work with OWH to design meeting track, or multiple sessions focusing on women's health c. Seek out partnerships and collaboration to support meeting d. Draft agenda e. Draft promotion plan Task 6: National Meeting evaluation a. Work with OWH to determine evaluation criteria for women's health track b. Develop an evaluation instrument c. Data collection for evaluation at completion of program d. Draft evaluation report e. Finalize evaluation report Task 7: Final report a. Draft final report summarizing the evaluation of program and annual meeting and lessons learned b. Finalize report c. Present findings to OWH 2.5 PERSONNEL AND CONTRACTOR REQUIREMENTS The Contractor shall ensure that all personnel assigned to work on the project are qualified to perform the tasks required in this SOW. 2.5.1 Contractor Requirements: The contractor should: • Be a membership-based independent, professional development-oriented journalism organization. Have members who are devoted to reporting and editing about health promotion, disease prevention, and health care issues • Have membership that encompasses all types of news media outlets (print, broadcast, Internet, and social media) ranging from large, medium, to smaller circulation • Have membership from diverse local, regional, national, and international news organizations within the U.S. and other nations • Have membership comprised of students, younger, mid-career, and veteran health and medical journalists • Encourage professional training and development for health and medical journalists (including but not limited to minority journalists) through ongoing and special programs within the organization • Encourage news coverage of medically underserved audiences and vulnerable populations • Encourage and critique news coverage of health and medicine • Have a website which includes information on health and medical care journalism issues and covers organizational services for members • Have an annual or biennial meeting for its members to educate them on relevant issues with an emphasis on the coverage of health issues within the United States. • Have membership criteria, professional standards, bylaws, statement of principles, conflict of interest policy and fundraising policies that boost the organization's credibility and stature among news media professionals including senior news media executives, whose assistance is needed to provide leave time for the selected program attendees • Provide ongoing professional training programs for its members, creating possible gateway for program attendees to train members in the use of OWH resources • Have experience in working with the Federal Government 2.5.2 Requirement to Notify Government of Proprietary Work Dependencies: Offerors are required to notify the Government in writing of any dependencies of the deliverables under this contract on proprietary, copyrighted, or patented work that potentially inhibits, restricts, or requires permission for the dissemination of the deliverables to the public, other governmental agencies or research groups, or any other parties whatsoever. SECTION 3 - PACKAGING AND MARKING 3.1 PACKAGING AND MARKING All deliverables shall be delivered to the Contracting Officer's Representative (COR) identified in Section 6 and shall be marked as follows: 1. Name and address of the Contractor; 2. Contract Number; 3. Description of item contained therein; and 4. Consignee's name and address. SECTION 4 - INSPECTION AND ACCEPTANCE 4.1 INSPECTION AND ACCEPTANCE (Commercial) Pursuant to 52.212-4, all work described in the SOW to be delivered under this contract is subject to final inspection and acceptance by an authorized representative of the Government. The authorized representative of the Government is the Government's COR, who is responsible for inspection and acceptance of all services, materials, or supplies to be provided by the Contractor.. SECTION 5 - DELIVERIES OR PERFORMANCE 5.1 PERIOD OF PERFORMANCE The Period of Performance for this contract is for 12 months from September 30,2018 through September 30,2019, and two option periods. The term "technical direction" includes, without limitation, direction to the contractor that directs or redirects the labor effort, shifts the work between work areas or locations, fills in details and otherwise serves to ensure that tasks outlined in the work statement are accomplished satisfactorily. (b) Technical direction must be within the scope of the specification(s)/work statement. The Contracting Officer's Representative does not have authority to issue technical direction that: (1) constitutes a change of assignment or additional work outside the specification(s)/statement of work; (2) Constitutes a change as defined in the clause entitled "Changes"; (3) In any manner causes an increase or decrease in the contract price, or the time Required for contract performance; (4) Changes any of the terms, conditions, or specification(s)/work statement of the Contract; (5) Interferes with the contractor's right to perform under the terms and conditions of the Contract; or (6) Directs, supervises or otherwise controls the actions of the contractor's employees. (c) Technical direction may be oral or in writing. The Contracting Officer's Representative shall confirm oral direction in writing within five work days, with a copy to the Contracting Officer. (d) The contractor shall proceed promptly with performance resulting from the technical direction issued by the Contracting Officers, Representative. If, in the opinion of the contractor, any direction of the Contracting Officers, Representative, or his/her designee, falls within the limitations in (b), above, the contractor shall immediately notify the Contracting Officer no later than the beginning of the next Government work day. (e) Failure of the contractor and the Contracting Officer to agree that technical direction is within the scope of the contract shall be subject to the terms of the clause entitled "Disputes."   6.3 INVOICE SUBMISSION - COMMERCIAL The Contractor shall submit invoices once per month. A complete invoice with all required back-up documentation shall be sent electronically, via email, to: 1. Contract Specialist via DAM mailbox: HQInvoices@ihs.gov 2. Financial Management Service (FMS): HQInvoices@ihs.gov * No other non-invoice related documents (i.e. deliverables, reports, balance statements) shall be sent to the DAM and FMS mailboxes. Invoices Only. The subject line of your email invoice submission shall contain the contract number, contract line item number, the order number, if applicable, and the number of invoices. The Contractor shall send one email per contract upon delivery and acceptance of services. The email may have multiple invoices for the contract. Invoices must be in the following formats: PDF, TIFF, or Word. No Excel formats will be accepted. The electronic file cannot contain multiple invoices; example, 10 invoices requires 10 separate files (PDF or TIFF or Word). Invoices shall be submitted in accordance with the contract terms, i.e. payment schedule, progress payments, partial payments, deliverables, etc. All calls concerning contract payment shall be directed to the COR. In accordance with FAR 52.212-4, Contract Terms and Conditions-Commercial Items, a proper invoice must include the following items: FAR 52.212-4(g) Invoice. (1) The Contractor shall submit an original invoice and three copies (or electronic invoice, if authorized) to the address designated in the contract to receive invoices. An invoice must include: (i) Name and address of the Contractor; (ii) Invoice date and number; (iii) Contract number, contract line item number and, if applicable, the order number; (iv) Description, quantity, unit of measure, unit price and extended price of the items delivered; (v) Shipping number and date of shipment, including the bill of lading number and weight of shipment if shipped on Government bill of lading; (vi) Terms of any discount for prompt payment offered; (vii) Name and address of official to whom payment is to be sent; (viii) Name, title, and phone number of person to notify in event of defective invoice; and (ix) Taxpayer Identification Number (TIN). The Contractor shall include its TIN on the invoice only if required elsewhere in this contract. (x) Electronic funds transfer (EFT) banking information. (A) The Contractor shall include EFT banking information on the invoice only if required elsewhere in this contract. (B) If EFT banking information is not required to be on the invoice, in order for the invoice to be a proper invoice, the Contractor shall have submitted correct EFT banking information in accordance with the applicable solicitation provision, contract clause (e.g., 52.232-33, Payment by Electronic Funds Transfer, System for Award Management, or 52.232-34, Payment by Electronic Funds Transfer, Other Than System for Award Management), or applicable agency procedures. (C) EFT banking information is not required if the Government waived the requirement to pay by EFT. (2) Invoices will be handled in accordance with the Prompt Payment Act (31 U.S.C. 3903) and Office of Management and Budget (OMB) prompt payment regulations at 5 CFR Part 1315. Additionally, the FMS requires the contractor to include its Dun & Bradstreet Number (DUNS) on each invoice. In accordance with OMB Memorandum, M-11-32, Agencies shall make payments to small businesses as soon as practicable, with the goal of making payments within 15 days of receipt of a proper invoice. If a small business contractor is not paid within this (15 day) accelerated period, the contractor will not be given a late-payment interest penalty. Interest penalties, as prescribed by the Prompt Payment Act, remain unchanged by means of this memorandum. All small businesses shall label all invoices as "Small Business." Additionally, in accordance with OMB Memorandum, M-12-16, all prime contractors are encouraged to disburse funds received from the Federal Government to their small business subcontractors in a prompt manner. To assist prime contractors in expediting contractor payments to small business subcontractors, Agencies shall, to the full extent permitted by law, temporarily establish an earlier, accelerated date for making agency payments to all prime contractors. Consistent with OMB Memorandum M-11-32 above, Agencies shall have a goal of paying all prime contractors within 15 days of receiving proper documentation. In an effort to support small business growth, drive economic activity and job creation, the Contractor is encouraged to accelerate payments to their small business subcontractors. In accordance with the requirements of the Debt Collection Improvement Act of 1996, all payments under this order will be made by electronic funds transfer (EFT). The Contractor shall provide financial institution information to the Finance Office designated above in accordance with FAR 52.232-33 Payment by Electronic Funds Transfer - System for Award Management. SECTION 7 - SPECIAL CONTRACT REQUIREMENT 7.1 PROHIBITION AGAINST PERSONAL SERVICES The Contractor shall not perform personal services under this contract. Contractor personnel are employees of the Contractor or its subcontractors and are under the administrative control and supervision of the Contractor. A Contractor supervisor must give all individual Contractor employee assignments and daily work direction. The Government will not supervise or direct Contractor employees in the performance of their assignments. If at any time the Contractor believes that any Government action or communication has been given that would create a personal service relationship between the Government and any Contractor employee, the contractor shall promptly notify the Contracting Officer of this communication or action. The Contractor shall not perform any inherently-governmental functions under this contract. No Contractor employee shall represent or give the appearance that he/she is a Government employee, agent or representative. No Contractor employee shall state orally or in writing at any time that he or she is acting on behalf of the Government. The Contractor is responsible for ensuring that all employees assigned to this contract understand and are committed to following these requirements.   7.2 RESTRICTIONS ON DISCLOSURE OF INFORMATION Information made available to the contractor by the Government for the performance or administration of this effort shall be used only for those purposes and shall not be used in any other way without the written agreement of the Contracting Officer. The contractor agrees to assume responsibility for protecting the confidentiality of Government records, which are not public information. Each contractor or employee of the contractor to whom information may be made available or disclosed shall be notified in writing by the contractor that such information may be disclosed only for a purpose and to the extent authorized herein. The contractor shall not use, disclose, or reproduce proprietary data that bears a restrictive legend, other than as required in the performance of this effort. Nothing herein shall preclude the use of any data independently acquired by the contractor without such limitations or prohibit an agreement at no cost to the Government between the contractor and the data owner which provides for greater rights to the contractor. The Contractor agrees not to release or disclose, verbally or in writing, information pertaining to the results or findings of work (including data collection, analyses, draft or final papers and reports) for the period of this contract without first notifying the COR in writing at least 21 days prior to the release or disclosure. At a minimum, the notification shall include: (1) who or what is generating the request for advance information; (2) when and how project results/information would be released; and (3) what information would be released. "Place of manufacture" means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture. "Predecessor" means an entity that is replaced by a successor and includes any predecessors of the predecessor. "Restricted business operations" means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). The offeror [_] does [_] does not certify that- (i) The services under the contract are offered and sold regularly to non-Governmental customers, and are provided by the offeror (or subcontractor in the case of an exempt subcontract) to the general public in substantial quantities in the course of normal business operations; (ii) The contract services will be furnished at prices that are, or are based on, established catalog or market prices (see FAR 22.1003-4(d)(2)(iii)); (iii) Each service employee who will perform the services under the contract will spend only a small portion of his or her time (a monthly average of less than 20 percent of the available hours on an annualized basis, or less than 20 percent of available hours during the contract period if the contract period is less than a month) servicing the Government contract; and (iv) The compensation (wage and fringe benefits) plan for all service employees performing work under the contract is the same as that used for these employees and equivalent employees servicing commercial customers. SECTION 10 - INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFEROR 10.1 INQUIRIES/SUBMISSION All questions/inquiries concerning the solicitation document must be submitted by e-mail (no faxes or telephone calls please) to the Contract Specialist, no later than June 15, 2018, at NOON. By submitting a proposal in response to this solicitation, it is understood that your proposal shall become a part of the official contract file. SECTION 11 EVALUATION FACTORS FOR AWARD 11.1 52.252-1 SOLICITATION PROVISIONS INCORPORATED BY REFERENCE. (FEB 1998) This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. Also, the full text of a solicitation provision may be accessed electronically at this/these address(es): http://www.acquisition.gov/far/ 11.2 GENERAL In order for quotes to be considered acceptable and eligible for evaluation, they must be prepared in accordance with and in compliance with the instructions given in this RFQ and address all of the requirements set forth in the Statement of Work/Description of Requirement. Quotes which merely offer to conduct a program in accordance with the requirements of the Government's scope of work will not be eligible for an award.   11.3 EVALUATION CRITERIA The technical proposal will be scored against the evaluation criteria described below. Each factor shall be weighted as shown. The total number of possible points is 100. Proposals shall be no longer than 20 pages, double-spaced, using 12 point font with 1" margins. Consideration will be given for clarity, style, and format of the proposal. 11.3.1 TECHNICAL EXPERTISE AND APPROACH (30 POINTS): Describe the proposed approach to meet each of the requirements that the Contractor will be responsible for performing as specified in the tasks in the Statement of Work (SOW). The proposal is consistent with the stated project purpose. The proposed approach assures that the achievement of timely and satisfactory performance is feasible and reasonable. The proposal demonstrates that the Contractor is willing to commit its resources to successfully fulfilling the requirements of the SOW. The Contractor proposes realistic timeframes and identifies the chronological sequence by which key activities will occur in order to assure the completion of each task that the Contractor will be responsible for performing. 11.3.2 PERSONNEL AND STAFF QUALIFICATIONS (30 POINTS): The proposed staff should meet the requirements laid out in the "contractor requirements" section of the Statement of Work.
