DEBAKEY VA MEDICAL CENTER (VAMC), HOUSTON, TEXAS REQUEST FOR INFORMATION 36C25618Q9547 BACKGROUND: The Houston VAMC has a requirement for the following brand name supplies: SEE BELOW Manufacturer Stock # Item Description SEE SOW ATTACHED REQUESTED RESPONSES: The intent of this Request for Information is to establish sources to define the procurement strategy (e.g set-aside, sole source, unrestricted) for a solicitation that VA intends to post in the near future. Please respond to this RFI if you can provide supplies that can be determined EQUIVALENT to the products listed in the table in the background section above. Please provide details on the proposed EQUIVALENT products such as Manufacturer Name, Part Number, and Description. If you have an existing GSA or VA, Federal Supply Schedule contract, please include the contract details in your response. Please note that VA is particularly interested in determining the availability of Small Business Manufacturers. Respondents are solely responsible for all expenses associated with responding to this RFI. SCOPE OF WORK AND GENERAL SPECIFICATIONS FOR NEUROMOD TRANSCRANIAL ELECTRICAL STIMULATION (tXES) DEVICE Background: Transcranial direct-current stimulation (tDCS) is a noninvasive, nonconvulsive, well-tolerated and low-cost brain stimulation intervention. It involves the use of weak electric currents (1 3 mA), passed through brain tissue via electrodes placed on the scalp and is virtually imperceptible to the subject. One of the most promising applications of tDCS has been in the treatment of deperession and is also known to be effective in reducing refractory symptoms such as hallucinations and negative symptoms in schizophrenia. The NeuroMod transcranial electrical stimulation (tES) system can be used like any other table top tES device where the waveform is specified by the user and can be started with the push of a button on the front. The device is capable of all common types tES including transcranial direct current stimulation (TDCS), transcranial alternating current stimulation (TACS) and transcranial random noise stimulation (TRNS), thus giving it the name tXES. This device compares favorably to the already approved Non Significant Risk, Soterix and NeuroConn Stimulators. This Multi-channel stimulator enables stimulation at multiple brain sites and has added features for safety, comfort, coordination among multiple outputs, tamper resistance and ease of replication of stimulation across sessions. By continuously monitoring electrode impedance, it can detect insufficient contact with the skin and automatically terminate stimulation, thus reliably excluding any injury to the subject. The stimulator comes with 8 fully independent, programmable channels, each of which, can deliver up to 2 mA of stimulation. Supply Specifications Requirements: Physical Characteristics the contents of the NeuroMod tXES kit. A: The NeuroMod tXES Stimulator. B: The NeuroMod tXES Head Box. C: NeuroMod tXES Electrodes. Warranty and Support: The NeuroMod tXES Stimulator is warrantied for two years from the date of delivery. The NeuroMod Electrodes are warrantied for 90 days from the date of delivery. During the period of warranty, of the NeuroMod tXES Stimulator and Electrodes, technical support is included in the price of purchase. Delivery Location: Michael E. DeBakey VA Medical Center 2002 Holcombe Boulevard Houston, TX 77030 3. Installation and Training: Basic training by video conference is included in the purchase of the stimulator. Installation is minimal and can be done during the training session. Additional and advanced training for the NeuroMod tXES system by phone, videoconference, or site visit are available. 4. Operations and Storage Areas: All staff shall confine operations (including storage of materials) on Government premises to areas authorized and approved by the Contracting Officer and/or COR. Working space and space available shall be as determined by the COR. Staff are subject to rules of the MEDVAMC applicable to their conduct. Staff shall execute work so as to interfere as little as possible with normal functioning of the MEDVAMC as a whole, including operations of utility services, fire protection systems and any existing equipment, with work being done by others. Staff will not store materials and equipment in other than the designated contractor storage areas. Staff shall keep work, storage, and staging areas clean and neat. 5. Protection of Existing Structures, Equipment, Utilities, and Improvements: IT IS VERY IMPORTANT THAT ESSENTIAL AND LIFE SAFETY SYSTEMS BE CONTINUOUSLY MAINTAINED AND NOT INTERRUPTED WITHOUT TWO WEEKS PRIOR WRITTEN NOTICE AND APPROVAL FROM THE VA MEDICAL CENTER. 6. Material Safety Data Sheet (MSDS): Staff shall provide three (3) copies of each Material Safety Data Sheet for every product used on this project. MSDS sheets shall be provided for any material on the same day those materials arrive on VA property. At no time shall the staff have, or permit other personnel to have, materials on station without MSDS sheets. All instructions for use shall be followed. Products will not be used until MSDS's are submitted to the COR. Staff shall maintain a current, green in color, loose-leaf notebook on the job site at all times, which is readily available for viewing by the COR or VA Safety Officer. 7. Work Hours: Normal business hours are 7:30AM to 4:30PM Monday thru Friday excluding Federal Holidays. Work completed outside this time must be requested through the COR and approved by the CO. 8. ID Badges: All personnel are required to obtain a time-limited I. D. badge from the VA Police Service located in the main building, room 1A-74. This badge must be worn at all times when workers are on site. 9. Estimates: Vendors will submit one quotation for the purchase of Neuromod tXES Stimulator and match the requirements outlined in this RFQ. In the quotes, vendors will indicate whether their solution has MET, NOT MET or MET WITH QUALIFICATIONS for each line item in this document (MET WITH QUALIFICATIONS will require a detailed listing of non-compliance). In addition, vendors will complete the Instructions to Offertory s (ITO) and respond in detail to each question. Vendors are required to respond to each item individually in both documents. Include the specification/question number and text in your response document to facilitate review.
