This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.   This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.  Solicitation number 36C24818Q0778 is issued as a Request for Quotation (RFQ).   This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97, effective 01-24-2018. The associated North American Industry Classification System (NAICS) Code for this acquisition is 325412, size standard 1250 employees. Description of Requirement: Line Item 1: Total Parenteral Nutrition (TPN) Intravenous (IV) Bags, A list of contract line item number(s) can be found on Attachment A, as supplemented by the Statement of Work (SOW) found at Attachment A.   All deliveries are to be performed and accepted at the Department of Veteran Affairs (VA), James A Haley Veteran s Hospital, 13000 Bruce B Downs, Tampa, FL 33612. FOB: Origin The following Provisions applies to this acquisition: Provision 52.212-1, Instructions to Offerors Commercial Items. Offerors must submit their technical and management approach that addresses all requirements pertaining to the required services. Provision 52.212-2, Evaluation Commercial Items will not be used. The comparative evaluation procedures will be used in accordance with FAR 13.106-2(b)(3). Provision 52.212-3, Offeror Representations and Certifications Commercial Items. To be eligible for award offerors must be registered in System for Award Management (SAM) prior to award in accordance with the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items.
