All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Responses to this notice must be submitted in writing via email and must be received not later than Tuesday, May 29, 2018, 4:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing, and received not later than Tuesday, May 29, 2018, 4:00pm EST. All interested parties must respond to any related solicitation announcement separately from the responses to this announcement. Email: Delfo.Saco-mizhquiri@va.gov. STATEMENT OF WORK (SOW) Well Water Testing and Water Quality Report There are 5 wells, a treatment plant and 5 separate pump buildings, air stripper plant, two transfer pumps, chlorine injection system, 300,000 gallon water tower, and distribution throughout the 268-acre campus. The potable water and distribution system on campus shall specifically meet all the requirements of the NY State Department of Health Sanitary Code for a Community Water Supply System. Operational sampling and tests shall be performed in accordance with NY State requirements. All off site independent laboratory testing shall be complete and performed and submitted on schedule. All back flow preventers are to be tested annually in accordance with NY State requirements. 1. Well Water Testing & Water Quality Reports All testing must be in accordance with yearly minimum monitoring requirements for the Northport VAMC and the most current EPA Clean Water Act Standards. The Northport VAMC is its own water district and well water testing is mandatory to be performed. Sampling and testing of five drilled wells, distribution points, air strippers, and entry points consists of, but is not limited to: Well water samples are required to be obtained and tested monthly. Collect and analyze one water sample for Coliform bacteria from Buildings 10, 21, & 65 at the beginning of the month, no later than the seventh calendar day of the month. Sample shall be tested for residual chlorine. Collect and analyze one water sample for Coliform bacteria from Buildings 15, 63, 92, and 200 at the end of the month, no earlier than the twenty-third calendar day of the month. Sample shall be tested for residual chlorine. Collect and analyze one water sample for Coliform bacteria from Well numbers 1, 2, 3, 4, & 5 once a quarter in March, June, September, and December. Collect and analyze one water sample for inorganic chemicals from Well numbers 1, 2, 3, 4, & 5 once a year in June. Collect and analyze one water sample for inorganic chemicals from Buildings 200 once a year in June. Collect and analyze one water sample from each stripper tower for Coliform bacteria and Principal Organic Contaminants from Building 220 (Air Stripper Plant) once a quarter in March, June, September, and December. Collect and analyze one water sample for Principal Organic Contaminants from Well numbers 1, 2, 3, 4, & 5 once a quarter in March, June, September, and December. Collect and analyze one water sample for perchlorate from Well numbers 1, 2, 3, 4, & 5 once a year in June. Collect and analyze one water sample for Nitrate/Nitrite from well numbers 1, 2, 3, 4, & 5 once a quarter in March, June, September, and December. Collect and analyze one water sample for Nitrate/Nitrite from Building 200 during the month of June. Collect and analyze one water sample for Nitrate/Nitrite from Tower 1 and Tower 2 (Point-of-Entry) once a quarter in March, June, September, and December. Collect and analyze one water sample for disinfection byproducts from Building 65, Room 104 (Bathroom sink), once a year in August. Collect and analyze one water sample from Monskey Field once per month in May, June, July, August, and September. Collect and analyze twenty water samples for lead and copper concentrations every three (3) years; Last sampling was in 2016; Next sampling is due in 2019. Collect and analyze one water sample for asbestos from a tap served by asbestos cement pipe and under conditions where asbestos contamination is most likely to occur. This sampling is required every nine (9) years; Last sampling was on 8/19/2013; Next sampling is due by 8/19/2022. Collect and analyze one water sample for radiological from Well numbers 1, 2, 3, 4, & 5. Well numbers 2 and 4 were last sampled in 2014; Next sampling is due in 2020. Well number 3 was last sampled in 2011; Next sampling is due in 2020. Well number 1 was last sampled in 2014; Next sampling is due in 2023.
