- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement and attachments constitute the only solicitation; quotes are being requested. The solicitation is 36C24418Q0370 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. This solicitation is a 100% Service-Disabled Veteran-Owned Small Business Set-Aside. The applicable NAICS code is 339112 and the small business size standard is 750 employees. The Department of Veterans Affairs, Lebanon VA Medical Center, Lebanon, PA 17042, has a Brand Name Only requirement for the supply, delivery and assembly as needed of a total of 8 each of the Performa 3-Section X Frame Treatment Tables w/Power Flex, Wheels, Color Black.
