Description
CHAPLAIN-LED FAMILY RESILIENCY RETREAT CHILDCARE STATEMENT OF WORK 20-22 April 2018 Location: To Be Determined •1. DESCRIPTION OF SERVICES: To provide all labor and equipment necessary to perform the services specified for the items in paragraph 2. •2. SERVICE DELIVERY SUMMARY: •a. Scope of Work: To provide Childcare for the Family Resilience Retreat with an Estimation 47 children. •b. Itinerary : •1. Friday 20 April: Provide childcare 1815- 2045 •2. Saturday 21 April: Provide childcare 0815-1145 and 1315 -1900. Dinner Saturday will be during childcare. Hotel will have the dining area close to other guests open for exclusive use of the children 1700-1730 in buffet style. Childcare workers will escort children to and from dining room and assist as needed for younger children at the buffet. •3. Sunday 22 April. Provide childcare 0845-1145 •c. Requirements for childcare ratio standards: Age Maximum Child:Staff Ratio Maximum Group Size ≤ 12 months 3:1 6 13-35 months 4:1 8 3-year-olds 7:1 14 4-year-olds 8:1 16 5-year-olds 8:1 16 6- to 8-year-olds 10:1 20 9- to 12-year-olds 12:1 24 •d. Requirements Childcare : •1. Contractor shall be First aid and CPR qualified. •2. Contractor shall be able to accommodate one autistic child. •3. Contractor shall provide all personnel, labor, supervision, coordination, transportation, and resources necessary. •4. Contractor shall be able to accommodate kids with special needs. •5. Contractor will provide a copy of their certificate of insurance. •6. Contractor will provide a copy of their state licensing. •7. Contractor will supply toys, crafts and play items to include AV for each appropriate age group for use during childcare. •8. Contractor will provide their own meals, lodging, and parking. •9. Contractor will not be responsible for providing any medication to children. Medication will be the responsibility of the parents. •10. The Contractor shall provide a Project Manager to act as his or her representative with the government for all matters pertaining to the contract. The Contractor shall provide the name and telephone number of this individual for scheduling and coordination of services. The name and telephone number of the designated individual will be provided to the Contracting Officer and the Contracting Officer's Representative (COR) at the start of the contract period. •11. The Contractor shall maintain a quality control system that will ensure the quality of services and materials provided meets or exceeds contract requirements. •12. The contractor will need to coordinate with the venue NLT 2 weeks prior to the date of execution if time permits. •13. The Contractor will need to comply will all regulations for Child Development Services as laid out in AR 608-10. •e. Requirements Contractor: •1. CONTRACTOR CONSENT TO BACKGROUND CHECKS: •a. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified by Law Enforcement Agency Data System (LEADS) through the National Crime Information Center. LEADS checks will verify if a person is wanted by local, state, and federal agencies. •b. All contractor and subcontractor personnel must consent to LEADS background checks. Contractor and subcontractor personnel who do not consent to an LEADS check will be denied access to the installation. Information required to conduct an LEADS check inc l udes: full nam e, driver's license number, and/or social security number, date of birth of the person entering the installation, and completion of a background check questionnaire. The contractor shall provide this information using the Scott AFB Form 21, Contractors Consent for Background Check, and shall submit it in conjunction with the contractor's request for either base or vehicle passes. Completion of a successful LEADS check does not invalidate the requirement for an escort when contractor or subcontractor personnel are working within controlled or restricted areas. •c. Contractors shall ensure their employees and those of their subcontracts have t he proper credentials allowing them to work i n the United States. Persons later found to be undocumented or illegal aliens will be remanded to the proper authorities. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this clause. Furthermore, nothing in this clause s hall excuse the contractor from proceeding with the contract as required. •2. BADGES: •a. The contractor is required to provide identification badges for their employees. All contractor personnel shall wear these badges while on duty on the government site. Badges are required to identify the individual, company name, and be clearly and distinctly marked as contractor. Size, color, style, etc. are to be mutually agreed to by contractor and government. The contractor's identification badge will not be used as an entry requirement for installation entry or into any government designated controlled or restricted are
