Description
Award Criteria Breakdown IEE FORM Clauses Provisions Statement of Work MAJOR DUTIES AND RESPONSIBILITIES Performs the following work: 1.Contractor shall provide safety data sheets for all items (i.e., chemicals, materials, etc.) required for pest control. 2.Contractor shall provide monthly pest control (rodents, bats, ants, cockroaches, occasional invaders, and other pests identified by contractor) services for the White Earth Health Center, Naytahwaush Health Center, and Pine Point Health Center. 3.Contractor shall provide semi-annually one-shot (spiders, Asian beetles, flies, and other pests identified by the contractor) services for the White Earth Health Center, Naytahwaush Health Center, and Pine Point Health Center. 4.Contractor shall provide a written monthly pest activity report to the Contracting Officer's Representative. 5.Contractor shall obtain all necessary permits, licenses, zoning, and other federal, state, local, or tribal authorizations required for performing services related to this agreement and make copies available of such up request. 6.Contractor shall pay and process all fees assessed relating to the service provided. KNOWLEDGE REQUIRED 1.Contractor shall have knowledge of established professional pest control and one-shot practices. 2.Contractor shall have knowledge of proper use of various pieces of equipment. 3.Contractor must have knowledge of safety and infection control requirements. 4.Contractor shall have knowledge of all necessary permits, licenses, zoning, and other federal, state, and tribal authorizations required for performing services relating to the agreement. 5 Year Contract (Base year plus 4 option years)
