Description
Statement of Work (SOW) Table of Contents CUS 18 Request for Quote Pricing Sheet (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is 70B03C18Q00000054 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 541940 Veterinary Services. (3) This procurement shall be Open Market. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest-priced technically acceptable quote in compliance with the Statement of Work (SOW). Includes regular and reoccurring veterinary care for its US Government-owned detector dogs stationed in and around El Paso, Texas. These CBP canines are required to receive routine vaccinations and physical exams, as well as receive other necessary veterinary treatment and emergency medical services. Continuity and quality of service and consistent veterinary care for the individual CBP canines is of utmost importance. The number of canines to be served is 100. This number may fluctuate with canine assignments. The canines work address is: Customs and Border Protection El Paso Field Office, 9400 Viscount Blvd. El Paso, Texas 79925 (4) The Department of Homeland Security (DHS), Customs and Border Protection (CBP) intends to award a contract for the procurement of Veterinary Services. It is required that all contractors submitting a quote review the attached STATEMENT OF WORK to review all requirements. All contractors submitting a quote shall be required to meet the requirements found in the STATEMENT OF WORK. CBP reserves the right to award with or without a site visit to the contractor's facility, as well as make unscheduled inspections post-award to ensure acceptable conditions and that all requirements listed in the SOW are being met. (5) The purpose of this procurement action is to procure Veterinary Services on an as-needed, daily basis in El Paso, Texas with a responsible contractor. The current requirement is for 100 dogs; however, this number may fluctuate depending on the requirements of the work, and the quantity of dogs may increase or decrease over the duration of the service period. Please see attached Statement of Work for details of the requirement. (6) Contractors must complete and sign Blocks 13, 14, 15, and 16 on page one of this SF 18 form (RFQ Packet). Contractors must submit the first page of the RFQ packet, as well as, the attached quote form for pricing on specified items covered under Veterinary Services. Quotes that are incomplete, unsigned, or late will not be considered. In order to be considered for option years, please provide pricing for each year in the Veterinary Price Quote Form provided. (7) Veterinary Quotes will be evaluated on the prices for the following five elements: Office visit/physical exam; vaccines; dental cleaning; anesthesia and fecal exam for parasites. All other prices will be evaluated for price reasonableness. The Vet Price Quote Form is not intended to be an all-inclusive list of all services anticipated by Customs and Border Protection. It is focused on the standard and routine care services most often utilized by CBP. It serves only as an evaluation tool to compare pricing and analyze overall price reasonableness. Vendors who submit quotes must have a facility in existence at the time of submitting a quote in order to meet the inspection requirement.. In order to be considered for option years, please provide pricing for each year in the Vet Price Quote Form. Quotes that are incomplete, unsigned, or late will not be considered. (8) The vendor selected for award MUST register in System for Award Management (SAM) at https://www.sam.gov/portal/SAM/#1 and receive payment via Electronic Funds Transfer (EFT) as a condition of the contract. Registration in SAM requires a valid Dun & Bradstreet (DUNS) number. No award can be made to a vendor not registered in SAM. (9) Delivery of Submittals: Contractors must complete and submit the first page of the RFQ packet, as well as, the attached Veterinary price quote forms. Quote Forms and the first page of the RFQ packet must be RECEIVED through email or Fax by the Contract Specialist no later than March 21, 2018 at 5 P.M. Eastern Time (Indianapolis). No phone calls will be accepted. Late submissions will not be accepted. (10) Note: Intended Period of performance shall be a base year and four possible service year: Base Year: April 1, 2018 - March 31, 2019 Service Year 1: April 1, 2019 - March 31, 2020 Service Year 1: April 1, 2020 - March 31, 2021 Service Year 1: April 1, 2021 - March 31, 2022 Service Year 1: April 1, 2022 - March 31, 2023 Actual period of performance dates shall be outlined at award. (11) The Service Contract Act (SCA) Wage Determination (WD) 15-4233, Rev 3 and Executive Order (EO) 13658 are applicable to this requirement. (12) Wage Determination Information, (WDOL) is part of the Integrated Acquisition Environment, one of the E-Government initiatives in the President's Management Agenda. It is a collaborative effort of the Office of Management and Budget, Department of Labor, Department of Defense, General Services Administration, Department of Energy, and Department of Commerce.
