Description
STATEMENT OF WORK CARPET/FLOORING REPLACEMENT BUILDING 179 January 18, 2018 Part 1 GENERAL 1.01. LOCATION A. Location of work to be accomplished under this statement of work is at Vance Air force Base. Vance is located approximately three miles south of the city of Enid, Oklahoma just off U.S. Highway 81. All carpet replacement will take place in bldg. 179. 1.02. SCOPE OF WORK •A. The work covered under this statement of work shall consist of the following: Contractor will furnish all plant, labor, materials and equipment to perform all operations necessary to remove existing carpet from all areas which currently have carpet, ceramic tile and remove all cove base. Replace all carpet, ceramic tile and cove base with new materials. The amount of carpet to be replaced is 473.33 square yards, with 790 linear feet of cove base. •B. 1.03. SCHEDULE OF WORK A. Contractor will schedule each area in advance with the facility manager of bldg.179, Ms. Marcia Fuata. The contractor will move all furnishings from the areas scheduled, and replacing the same after the carpet is laid. All electronic equipment such as trainers and computers, monitors, and TVs will be moved by the facility occupants. B. Contractor will be required to remove all existing carpet and unusable cove base according to the established schedule, and replace with adhesive, new carpet, new ceramic tile and cove base. C. Contractor may choose to work some evenings and weekends. As long as the facility manager agrees to the schedule, this may be accomplished. D. Contractor will be liable for any and all replacement cost for furniture or facility damage caused by carpet installation. E. All freestanding furniture (files, chairs, tables, desks, etc) will be moved by contractor during carpet removal and replacement. 1.04. CONTRACTOR'S REPRESENTATIVE A. A representative of the contractor shall directly supervise accomplishment of this work. The representative shall be responsible for coordinating all phases of work with Contracting Office. The representative shall be on the job site or readily accessible at all times work is being performed and shall have authority to execute no cost field changes on behalf of the contractor if agreed to by all parties involved. B. This person will also be responsible for all quality control of the job site and will be held responsible by the Contracting Office for the removal and installation of all product in accordance with this statement of work. The representative shall be competent and familiar with all phases of work under his authority. The contractor's representative shall have in his possession a minimum of one pager or cell phone with local telephone number; this telephone number will be on record with Contracting Office. 1.05 PROVIDED SERVICES AND UTILITIES •A. Electricity: All reasonable required amount of electric power will be made available to the contractor by Contracting Office from the nearest suitable and available connection. •B. Water/Latrine: All reasonable required amount of water will be made available to the contractor. Latrine on location will be made available to installers. 1.06 INSPECTION A. The work will be conducted under the general direction of the Contracting Office and is subject to the inspection of the appointed inspectors to ensure strict compliance with the terms of the contract. 1.07 EXCESS MATERIALS (EXCLUDING HAZARDOUS MATERALS OR WASTE) •A. All waste materials resulting from accomplishment of this work shall become the property of the Contractor. The Contractor will haul all waste from this project off of Vance Air Force Base. No removed materials (carpet, cove base etc :) may be left outside of the facility during the hours the Contractor is not working. All waste materials shall be disposed of properly. •B. All left-over new material, i.e. carpet, cove base, adhesive, shall become the property of Vance Air Force Base. 1.08 SPECIAL MATERIALS A. Prohibited Material; The manufacturer of materials that are to be used in this contract has certified that the product does not contain mercury, toluene, chlorinated solvents, hydrolysable chlorine derivatives, ethylene base glycol ethers and their acetates, nor any carcinogen, as defined in 29 CFR 1910,1200. Materials used shall not contain lead and hexavalent chromium compounds. In addition, the Contractor shall comply with federal regulation 16 CFR-1303 in the use of lead, use on touch-up for damaged furniture or any interior surface of a facility. If lead based paint is used it shall be tested as specified ASTM-D 3335, the lead content shall not exceed 0.006 percent by weight of the dry film and the test for chromium content shall be negative. B. At no time shall class 1 Ozone Depleting Chemical (ODC's) be specified and shall not be used unless otherwise allowed by the Contracting Administrator 1.09. MATERIALS A. Materials to be provided through ASRC Local Purchasing. B. Safety Data Sheets (SDS) can be issued to the Contracting Office upon request. Part 2 EXECUTIONS 2.01 FLOOR PREPARTION / INSTALLATION A. All work will be completed within 30 business days of beginning the work. The process of replacing the carpet will be as nondestructive to the office environment as possible. B, The Contractor shall notify Contracting Office if electronic equipment/computers are not being removed from the scheduled location. Contractor will not be held responsible for removal or disconnecting any and all computers, telephones or other electronic equipment within work area. C. The Contractor working with the Contracting Office inspector will at all times have a working schedule as to areas/section he will be working in from day to day. D. Once carpet is in place, contractor will vacuum the new carpet surface removing any and all carpet pieces, string, and or other foreign matter. E. Cove base material shall be so used as to fabricate inside corners from rolled goods. F. All left-over new materials will become the property of Vance Air Force Base. END OF STATEMENT OF WORK
