- TUSCALOOSA VA Medical Center NCO 07 Contracting 3701 Loop Road East Tuscaloosa, AL 35404 December 21, 2017 Prospective Offerors Subject: Solicitation 36C24718Q0144, Relocation Services- Request for Quote Dear Prospective Offerors: The VA Medical Center invites your firm to submit a proposal for the subject requirement as indicated in the Price/Cost Schedule of the solicitation. The Government intends to make an award of a firm-fixed price contract for this procurement. Please submit a proposal in accordance with the following instructions: Complete blocks 12, 17a, 30(a) (b) and (c) of the attached SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the Solicitation. The Solicitation constitutes the model purchase contract. Complete Contractor information on page 3 of the solicitation Complete PRICE/COST SCHEDULE starting on page 13 of the solicitation Read the 1st paragraph of section E.2 on page 40 of the solicitation. Complete paragraph (b)(2) on page 43 by writing NONE in the blank if you completed the online reps and certs. Submit proposal on company letterhead also. Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation. Your quote shall be signed by an official authorized to bind the firm and be submitted on or before 2:00 p.m CST on Tuesday, December 26, 2017. All communications prior to award concerning the solicitation shall be directed to karen.dukes@va.gov.
