- STATEMENT OF WORK FOR PAWNEE CHEMISTRY ANALYZER LEASE The Indian Health Service has a requirement to rent/lease Chemistry Analyzer for the Pawnee Indian Health Center. Place of performance will be the Pawnee Indian Health Center, 1201 Heritage Circle, Pawnee, Oklahoma 74058. The government intends to award a firm fixed price contract for Base Year, plus four (4) option years for this requirement. Period of performance: base year, plus four (4) option years, approximate start date February 1 2018. The solicitation will be issued as Request for Quotes (RFQ) 246-18-Q-0005. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Contractor shall provide a firm, fixed-price rate for the requirement needed. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items. The RFQ will be issued as a full and open competition under North American Industry Classification System (NAICS) Code 423450 and a size standard of 200 employees. Vendor must have an updated SAM and ORCA record by the solicitation deadline to be considered for the award. It is the offeror's responsibility to register and/or confirm that their details are current. The closing date for receipt of quotes is January 5 2018. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on January 5 2018. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. No phone inquiries. Description of Items needed: The Pawnee Indian Health Center is a TJC accredited outpatient facility which provides comprehensive direct ambulatory care to rural Native Americans. There is a requirement at the health center for rental/lease of a Chemistry Analyzer for continued support of patient care requirements. Access to a chemistry analyzer is necessary to provide laboratory services to supplement the Health Center's direct patient care programs and to comply with TJC standards. Vendor shall furnish all labor, tools, parts, and materials for Chemistry Analyzer. Training, maintenance, call-outs and 24/7 telephone support will also be provided by the vendor. FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: 1) current registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/); 2) two relevant past performance references to include the contact name phone number, contract number, company name, brief description of project, dollar amount, and performance rating of that project; 3) pricing quote for cost-per-test lease of base year requirement, to include training, maintenance, and 24/7 support, as well as labor, tools, parts and materials for said chemistry analyzer. The Indian Health Service understands that pricing most likely cannot be furnished for period beyond the base year for the option year requirement. Award of purchase order does not guarantee that the Indian Health Service will exercise all four Option Years. FAR 52.212-2, Evaluation - Commercial Items applies to this acquisition. The Government (IHS) intends to award from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Past Performance; (2) Technical Acceptance; (3) Pricing. The following rating methods will be utilized for this requirement: Excellent, Good, Satisfactory, Marginal, and Unsatisfactory. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Jan 2017), applies to this acquisition. EVALUATION CRITERIA Technical as well as price will be a deciding factor for award of a purchase order. The due date for this requirement is January 5 2018 4:30 PM CT. THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THESE EVALUATION CRITERIA AS FOLLOWS: A) registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/. Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. B) Propose and provide pricing for Chemistry Analyzer lease for the Pawnee Indian Health Center. Estimated start date is February 1 2018, with an option of four (4) additional years.
