- The attached PWS is only a draft. Please direct all questions to the POC's listed in this synopsis. This announcement serves as notice to all potential offerors that the Illinois Army National Guard, USPFO, Contracting Office, 1301 N. MacArthur Blvd, Springfield, IL 62702 (IL-ARNG) anticipates the release of a formal Request for Proposal (RFP) under solicitation number W91SMC-18-R-0002 for Medical and Dental Periodic Health Assessments (PHAs). The government anticipates awarding a firm fixed price contract resulting from the RFP to the responsible offeror whose offer conforming to the RFP will be most advantageous to the government, price and other factors considered. The applicable North American Industry Classification System NAICS code is 621399, "Offices of All Other Miscellaneous Health Practitioners," with a size standard of $7.5 million. This solicitation will be issued as a Total Small Business Set Aside. The IL ARNG is advertising its intent to post the RFP on or around 03 January 2018, for a requirement meant for PHAs in support of the Illinois Army National Guard's medical and dental readiness. The Period of Performance is intended to begin 17 March 2018. This requirement will include the base year plus one option year. Services are to be completed on locations at various ILARNG Armories & Training Sites across the State. Further details will be provided within the solicitation. NOTE: All prospective contractors must be registered in System for Award Management (SAM) in order to be eligible for contract award. Registration may take up to three weeks to complete, so immediate attention to ensure eligibility is recommended. For registration, go to http://www.sam.gov and follow the instructions.
