- Required Submittal Checklist Scope of Work The Scope of Work has been revised to remove the requirement to provide a list of all contractor equipment. In accordance with the Scope of Work, 10. Contractor Submittals for Contracting Officer Approval, A. BPA List, The following submittals shall be reviewed for initial technical capability and ability to satisfy the requirements identified in the BPA: insurance certificate; and personnel list including current pesticide licenses (from Iowa, Illinois, Missouri, or Wisconsin however call orders may require licenses in up to all four states), proof of experience with example completed projects, and forester certifications. CONTRACTOR'S MUST PROVIDE THE FOLLOWING INFORMATION TO BE CONSIDERED FOR THE BPA: Completed checklist (attached) and the following information: SECTION 2 - SUPPLYING PLANTING MATERIALS: Proof of experience shall be furnished by the Contractor and approved by the Contracting Officer as part of the BPA. The Contractor must demonstrate experience through project lists or reports that this individual has conducted this work for at least 5 years. SECTION 3 - SITE PREPARATION: Proof of experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. SECTION 4 - ROUGH CUT MOWING: Proof of certification and/or experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must provide documentation of forester certification or demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. SECTION 5 - HERBICIDE SUPPLY AND APPLICATION: Proof of certification and/or experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must provide documentation of forester certification or demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. The Contractor must provide appropriate state pesticide applicator license(s). SECTION 6 - DIRECT SEEDING: Proof of experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. SECTION 7 - HERBACEOUS, SHRUB, AND TREE PLANTING: Proof of experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. Experience is to include at least two different examples of the planting stock types listed above in planting efforts in remote locations. The Contractor must demonstrate experience through project lists or reports that this individual overseeing the daily planting efforts has conducted this work for at least five years. SECTION 8 - HAND CREW CHAINSAW CUTTING: Proof of experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must provide documentation of forester certification or demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. The Contractor must provide appropriate state pesticide applicator license(s). SECTION 9 - FORESTRY MULCHING: Proof of certification and/or experience shall be furnished by the Contractor and approved by the Contracting Officer for the BPA. The Contractor must provide documentation of forester certification or demonstrate experience through project lists or reports that this individual has conducted this work for at least five years. CONTRACTORS DO NOT HAVE TO BE QUALIFIED FOR ALL SECTIONS IN ORDER TO BE A PART OF THE BPA. PLEASE ENSURE YOU INCLUDE THE SECTIONS YOU ARE RESPONDING TO IN YOUR RESPONSE.
