- CREDIT RELEASE PAST PERFORMANCE FORM ACH FORM RFP WORKBOOK SOLICITATION DOCUMENT This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number is CT2236-18 and this solicitation is issued as a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96. The 2012 North American Industry Classification System code is 337910, Mattress Manufacturing. UNICOR, Federal Prison Industries, Inc., intends to enter into a three (3) year with firm-fixed price Requirements type contract with EPA.
