Kingsbridge Road Rm. 9b-16 Bronx NY 10468 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 36C24218Q0117 and is issued as a request for quotes (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-61. The associated North American Industrial Classification System (NAICS) code for this procurement is 238210 with a small business size standard of 15 million. This requirement is [SB SET-ASIDE] and only qualified offerors may submit bids. The solicitation pricing on www.FBO.com will start on the date this solicitation is posted and will end on December 08, 2017 3:00Pm. Eastern Time. FOB Destination shall be Multiples shipping information. Delivery will be made within 60 days ARO to Northport VAMC 79 Middleville Road ATTN: John Derudder Engineering Service, Northport, NY 11768. This procurement is to provide and install 100 amp services for the police guard booth located at the Northport VAMC. Provide and install 100 amp services for the police guard booth located at the Northport VAMC. The Government purchases vehicle and bulk fuel oil for boilers. The Contractor is responsible for all aspects of petroleum fuel management including establishing and implementing a schedule for inspecting, maintaining, and repairing all petroleum storage facilities and systems to ensure safe and reliable delivery, transfer, storage, and use; and, continuous fuel service to boilers, heating systems, and emergency generators at VAMC Northport 24 hours a day, every day of the year. All petroleum use, storage, and maintenance of petroleum facilities must be performed in a manner, and on a schedule, consistent with applicable Federal, state and local regulatory requirements. The Contractor is responsible for all operations and maintenance of tanks, piping and appurtenances including piping, secondary piping, secondary containment pads, including but not limited to cleaning, rust-removal, and painting. These facilities are inspected at least annually by federal, state, and local regulators. These include but are not limited to: 40 CFR 112 (USEPA); Oil Pollution Act of 1990 (OPA 90) including 40 CFR 112 et al; Article 12 of the New York State Navigation Law; Title 6 New York Codes, Rules and Regulations Part 610 (6 NYCRR 610); Suffolk County Department of Health Services Article 12; New York State Department of Environmental Conservation (NYSDEC) Petroleum Bulk Storage/Major Oil storage Facility requirements; PIADC s Facility Response Plan/Spill Prevention, Control and Countermeasures (FRP/SPCC) plan as required by EPA's FRP rule 40 CFR 112; and, 33 CFR 154 (United States Coast Guard or USCG). The Contractor will manage, maintain, and inspect all leak detections systems, monitoring systems, and alarms, including but not limited to performing and submitting the following reports: Performing weekly inspections and reporting for fuel dispensers, above-ground fuel tanks (ASTs), and underground fuel tanks (USTs). Performing monthly inspections and reporting for truck fuel, ASTs, USTs, and boiler/generator day tanks. Performing bi-annual sampling and analyses of groundwater samples from on-site groundwater monitoring wells. Performing annual pipeline pressure test and reports. Performing 10-day diesel and gasoline use reconciliation statements. Perform other functions as new policies, procedures and regulations dictate. The contractor shall inspect all tanks including, but not limited to Gasoline, Diesel, #2 Fuel Oil, Motor Oil, Grease, Used Oil, Hydraulic Oil, and Mineral Oil on a monthly basis. Based on the inspection findings, the contractor shall prepare a monthly inspection report and submit to VAMC Northport GEMS Coordinator. The contractor shall prepare an annual air compliance report and submit to the VAMC Northport GEMS Coordinator and VAMC Northport COR. The contractor will carry out scheduled maintenance services to include the following: The contractor is to perform on a daily basis release detection of tanks and piping according to 40 CFR Part 280, Technical Standards and Corrective Action Requirement for Owners and Operators of Underground Storage Tanks (USTs). Daily monitoring of (1) 4000 gallon gasoline tank at the fueling point adjacent to Building T-127 to include the following: Gallons of Gasoline in the tank. Gallons of Gasoline consumed from the tank during each day. Height of gasoline in the tank. Height of water in the tank. Using the data gathered on a daily basis, prepare a 30-day reconciliation report. Calculate consumption of gasoline from the tank to determine if it equals the fuel draw from the tank. If determined the draw down from the tank exceeds the consumption, then inform VAMC Northport COR immediately for corrective action. Weekly monitoring of (1) 550 Gallon diesel tank at well house #2 and (1) 550 Gallon diesel tank at well house #3. Gallons of diesel in the tank to include, but not limited to the following: Gallons of diesel in the tank. Gallons of diesel consumed from each tank, weekly. Height of diesel in each tank. Height of water in each tank. Using the data gathered on a daily basis, prepare a monthly report. Calculate consumption of gasoline from the tank to determine if it equals the fuel draw from the tank. If determined the draw down from the tank exceeds the consumption, then inform VAMC Northport COR immediately for corrective action. Bid MUST be good for 60 calendar days after close of Buy.
