DeGen Medical, Inc. 1321 North Casua Drive Florence, SC 29501-6939 Products: P/N 0210-2030T, Hyper-C Plate, 2-Level 30mm 1 EA P/N 0210-2028T, Hyper-C Plate, 2-Level 28mm 1 EA P/N 02-01G12, Drill Bit 2.3, 12mm 2 EA P/N 02-03P212, Fixation Pin, Large, 12mm 2 EA P/N 0202-4214V, Bone Screws, 4.2 x 14mm 12 EA P/N 0302-0814L, Cervical Spacer, 8Deg, 8x14x12 1 EA P/N 0302-0714L, Cervical Spacer, 8Deg, 7x14x12 1 EA P/N 0302-0716L, Cervical Spacer, 8Deg, 7x16x14 2 EA These surgery parts were chosen because in the surgeon's opinion, it meets specific needs and requirement of the patient's condition and will provide the desired surgical outcome. These are specific products that are requested by the surgeons. These products are considered all or none and in the best interest of the patient. No telephonic request will be honored or returned.
