Description
SOURCES SOUGHT ANNOUNCEMENT: The Erie VAMC, is conducting a market survey to identify potential sources to provide ETO cleaning services. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The North American Industry Classification System Code (NAICS) is 811219 and the small business size standard is 20.5 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years or whether you have a socio-economic designation. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the Transportation of patients IAW the SOW. Sources are asked to provide information to demonstrate their answers to the following items of information: 1. Company Name, Address, Website, POC, and POC Phone Number. 2. Indicate and explain your capability of providing this service including maintaining any required security clearances and or additional licensing. 3. Provide a summary of the type transportation equipment that will be used and any relevant licensing. 4. If these services are available under your GSA contract, please provide the FSS number. 5. Please indicate your company's size under NAICS Code 811219. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments. Information regarding your firm's capacity and capability to provide such services are to be submitted electronically on or before Wednesday, 12/11/2017 prior to 4:00 pm (EST) addressed to Mark Andrews, Contract Specialist: Mark.andrews3@va.gov. Phone: 717-228-6018. STATEMENT OF WORK FOR Cleaning the Ethylene Oxide Sterilizer Chambers Background. The Erie VA Medical Center has a requirement for the chemical cleaning of the Ethylene Oxide Sterilizer Chambers. The NC-CAPA inspection team cited this facility for staining on the inside of the ETO sterilizer chambers. Scope. The contractor will utilize chemicals to remove the staining on the interior of the ETO chamber. The interior surface of the chamber must be returned, as much as possible, to the original condition. The contractor will provide a list of the chemicals to be used and the MSDS sheets, along with the submission of the quotation. The contractor must also provide references to show their experience with cleaning ETO sterilizers. Applicable Documents. Upon completion of the service the Contractor's representative shall inform the Contracting Officers Representative (COR) or the Point of Contact (POC) and provide a copy of the service report. The report shall include 1) date of service 2) equipment serviced 3) action taken 4) any costs not covered under the service contract 5) signature of authorized COR or POC official. Pre-contract Award Requirements. Prior to the award of the contract, the bidder will provide a list of the chemicals to be used and the MSDS sheets for those chemicals toe COR. The VAMC may need to send the list of chemicals to the manufacturer to ensure the chemicals will not alter, in any way the performance of the sterilizer, will maintain the integrity of the metal and not cause any reaction with the ETO during sterilization. All chemicals must be compatible with the metal construction of the chamber and cause no discoloration or pitting of the chamber. Prior to the award of the contract the bidder must provide a Work Safety Protection Plan, to include a list of the Personal Protection Equipment and Respiratory Protection equipment, if needed to the COR. Performance Requirements. The Contractor must perform the work no later than 4 weeks after the award of the contract. The contractor will perform the work on a weekend and between the hours of 7:00am and 10:00pm. Contractor will thoroughly rinse the chambers after using the cleaning products. Depending upon the hazardous qualities of the chemicals, the chemical waste and rinse water may need to be collected and disposed of properly. If the chemicals can t be flushed into the sterilizer drain, the appropriate VAMC technician will disconnect the sterilizer from the drain and provide a container for the collection of the chemical waste. Disposal of the waste will be the responsibility of the Erie VAMC. Any isolation barriers that might be required, will be assembled, installed and removed by the Contractor. Any chemicals brought into the facility but not used, will be removed by the Contractor. Place of Performance. The place of performance for this requirement is the Sterile Processing Service, located in the main building of the Erie VA Medical Center, 135 East 38 Street, Erie, PA 16504. Security. All contractor and subcontractor employees requiring access necessary to fulfill this requirement, shall, within one week of contract commencement, complete the VA Security/Privacy Awareness training on an annual basis while the contract is in place. All contractor and subcontractor employees being granted access to VA information or information systems shall sign an acknowledgement that they have read, understand and agree to abide by the VA s Contractor Rules of behavior.
