- Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation for this acquisition is IHS1341664. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This solicitation document and its incorporated provisions are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This requirement is a total Small Business set-aside. The North American Industry Classification System (NAICS) Code for this acquisition is 541611 - Administrative Management and General Management Consulting Services with a small business size standard of $15.0 million. The Contract Line Items (CLINs) and pricing structure are as shown below: CLIN 0001: Consultant Services for DVPI Projects -- $_______________ Offeror shall indicate above the Total Cost to perform stated services in accordance with the Statement of Work. Total Cost shall be inclusive of labor, travel, per diem, tools, equipment, technical, materials, supplies and federal/state/local taxes and all other necessary expenses to perform services in accordance with the attached Statement of Work. Delivery Location: Gallup Indian Medical Center, 516 E. Nizhoni Blvd., Gallup, NM 87301. FOB shall be destination, if applicable. The purpose of this fixed price purchase order is to secure the services of a consultant to implement and guide each Domestic Violence Prevention Initiative (DVPI) contracted project with the local data plan and other effective process and impact evaluation strategies. See the attached Performance Work Statement for more details. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. The provision at 52.212-2 Evaluation-Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Capability; Past Performance and Price. Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications - Commercial Items. FAR52.212-4, Contract Terms and Conditions-Commercial Items (JAN 2017), applies to this acquisition. (b) The Contractor shall account for all forms of Government-provided identification issued to the Contractor employees in connection with performance under this contract. The Contractor shall return such identification to the issuing agency at the earliest of any of the following, unless otherwise determined by the Government; (1) When no longer needed for contract performance. (2) Upon completion of the Contractor employee's employment. (3) Upon contract completion or termination. (c) The Contracting Officer may delay final payment under a contract if the Contractor fails to comply with these requirements. (d) The Contractor shall insert the substance of clause, including this paragraph (d), in all subcontracts when the subcontractor's employees are required to have routine physical access to a Federally-controlled facility and/or routine access to a Federally-controlled information system. It shall be the responsibility of the prime Contractor to return such identification to the issuing agency in accordance with the terms set forth in paragraph (b) of this section, unless otherwise approved in writing by the Contracting Officer.
