Page 1 of 3 Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs (VA), Hudson Valley Health Care System, Montrose Campus VA Medical Center is currently conducting a market survey for qualified firms capable of providing all labor, supplies, equipment, maintenance, information technology, and supervision necessary to provide hotel and conference room accommodations for approximately 25 people for Friday thru Sunday meetings as described herein. New York counties for consideration to hold the event are Westchester, Putnam, Duchess, Rockland and Orange. Program known as WARRIOR TO SOULMATE Retreats includes, but not limited to the following: VA HVHCS will contract with hotel contractor to hold a retreat for Veterans who receive services through VA HVHCS that have need for relationship education due to strained relationships. The retreats for Veterans on a range of issues, this retreat will be offered to Veterans who may have transition issues and/or are under significant relationship stress. The hotel contractor will provide accommodations (room with two queen-sized beds, or room with king-sized bed), for each participant couple and each workshop facilitator. Depending on the agenda arranged for the retreat, rooms would be required for either one or two consecutive nights, usually a Friday and Saturday night. Projected date for the retreat : March 1, 2018 Subsequent years, if option years are exercised, will follow a minimum of once per year schedule for subsequent fiscal years. The site (hotel) must have a conference room available for 8 hour per day, on the days in which the participants and facilitators are conducting the workshops. The space needs to comfortably accommodate a minimum of 25 people. VAHVHCS RESPONSIBILITIES: Provide names of attendees, and facilitators to hotel contractor, who will manage registration and room assignment. VAHVHCS will schedule date with the hotel site, based on needs of the VAHVHCS and dates on which the hotel has availability to accommodate the need for accommodations. VAHVHCS will provide payment for conference room, audio/visual rentals and food. CONTRACTOR RESPONSIBILITIES: Provide overnight accommodations, as described above, for each night s stay required. Have available a meeting or conference rooms appropriate for the workshop, of a size and shape to comfortably accommodate approximately 25 people, and any necessary audio-visual equipment with wireless microphone podium needed for the retreat workshops. Food shall be served/available as follows: Breakfast 8am Saturday and Sunday, lunch Noon Saturday, dinner 6 PM Friday and Saturday and breaks- 10am & 3pm Saturday. The North American Industry Classification Systems (NAICS) is 721110. Responses to this notice must be submitted in writing via email to carlton.lunsford@va.gov and must be received not later than December 6, 2017 at 3:00 pm EST. No telephone inquiries will be accepted. Interested firms should also add their information to the interested vendors list for this announcement at www.fbo.gov. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 721110. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include: company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, DUNS number, address, point of contact, phone number and socioeconomic category: (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business or Large Business). Additionally: If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractors must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov).
