REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. SOURCES SOUGHT: Veterans Health Affairs Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify homeless shelters/facilities that can provide services for homeless Veterans, including those with serious mental illnesses, substance abuse disorders, and/or medical conditions. REQUIREMENT: The Department Of Veterans Affairs, Erie VA Medical Center (Erie VAMC) is seeking a Contractor to provide emergency shelter services as part of its Community Based Health Care for Homeless Veterans (HCHV) program to referred Veterans. The Contractor shall be expected to provide short term, low barrier rapid residential placement that enhances the stability leading to permanent housing for Veterans and families. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. The target population is chronically homeless, chronically mentally ill Veterans, Veterans with chronic substance abuse disorders and complex needs. Eligibility will be determined by the VA Homeless Coordinator or designee, based on Veteran s assessment. The facility must be capable of passing an initial inspection conducted by select personnel from the VA. The Contractor shall provide all labor, supervision, housing, material and supplies necessary to provide emergency residential placement, treatment and supportive services through the HCHV program Safe Haven Model. Services will be provided on-site at the Contractor s facility. SERVICES TO BE PROVIDED: Contractor will be responsible for providing sufficient residential capacity to provide emergency shelter for up to three (3) Veterans per day in safe, ADA accessible, low barrier, community-based residences. Identification of suitable residential placement generally requires: a) sufficient personnel to assure security; b) staffing available on-site 24 hours a day, 7 days per week; c) 3 healthy meals per day; d) available no-cost laundry facilities for residents to do their own laundry; e) the ability to place a Veteran on the same day of a referral from HCHV staff; f) ability to serve both male and female Veterans (if families are a possibility, please also include this data). Basic services to include: Room and Board: Room and Board to include a bed and other furnishings such as a dresser, storage, and personal linens (towels and bed sheets). Dietetic Services: Meals to include at least three (3) nutritionally adequate meals a day, 7 days a week and availability of nutritious snacks Laundry Facilities: On-site laundry facilities and supplies Internet and Computer Access Environment of Care Services: The Contractor must provide a clean and comfortable environment of care that is structurally sound facility; does not pose any threat to the health and safety of the occupants and protects them from the elements. Transportation Supportive services to include: Structured group activities as appropriate or by referral to community resources where such services are accessible Collaboration with the VA program staff, which will provide supportive/therapeutic social services Assistance to develop responsible living patterns, to maintain an acceptable levels of personal hygiene and grooming, and to achieve a more adaptive level of psychosocial functioning Support for an alcohol/drug abuse-free lifestyle The North American Industry Classification System Code (NAICS) is 624221 and the small business size standard is $11.0 million RESPONSES: NCO 4 is seeking responses from contractors that are interested in this requirement and can demonstrate their capability to provide the required services.
