Page 5 of 5 Combined Synopsis Solicitation: Calendars 1. General: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-1215 and is issued as a Request for Quote (RFQ). This solicitation document and incorporates provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94,2005-95, effective January 19, 2017. This procurement is set-aside 100% for Small Business and only qualified Offerors may submit quotes. This requirement will be awarded on all-or-none basis. The applicable North American Industry Classification System Code (NAICS) is 323111 and the small business size standard is 500 employees. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer AND if the item is on a FSS/GSA schedule awarded to the Offeror include the schedule information on your offer. Quote must be good for 30 calendar days after close of this Combined Synopsis Solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. 2. Line items: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 2018 Cleveland VA Healthy You Calendar 54,000.00 EA GRAND TOTAL 3.Background: The Department of Veterans Affairs, Louis Stokes Cleveland Veterans Affairs Medical Center (CLE VAMC), has a request for 2018 Healthy You Calendars and a phone application with an eGuide. The planner and eGuide shall enhance the Veterans experience and promote healthy living in the community.   The planner highlights key areas of health concerns for Veterans using the National Center for Health Promotion and Disease Prevention 9 Healthy Living Messages. With a special emphasis on diseases and conditions affecting large Veteran demographics, the Healthy Living Planner empowers Veterans with relevant good health information. The Healthy Living Planner shall be dynamic in its ability to provide quick tips monthly for improved self-care and patient knowledge. It shall be embedded with codes to receive the complimentary copy of the VA Whole Health Journey digital resource. The Self-Management eGuide shall be from a collection of eGuides and pocket guides that support patients in taking charge of their health, achieving well-being, and succeeding in making positive health behavior changes. With the introduction of the Whole Health Partnership model at the Cleveland VAMC, implementation of health coaches and whole health facilitation groups, a suite of Veteran education materials is needed to assist health coaches, physicians, dietitians, and health facilitators in their work. The eGuide shall be able to be downloaded by patients to their smart phone or tablet to make them portable and widely accessible. The eGuide can also be used by providers at point-of-care to assist with patient education and skill building. They can also be given to patients to take home to encourage self-management. 4. Tasks: Task 1: Provide 2018 VA Healthy Living Planner Cleveland VAMC w/embedded codes in a smart phone and tablet application for free downloads. Task 2: Content of Calendar must include: VA Personalized Health Plan; VA 9 Healthy Living Messages spread out over a 12-month period including: Be Physically Active; Strive for a Healthy Weight; Be Involved in Your Healthcare; Limit Alcohol; Be Safe; Eat Wisely; Screening Tests & Immunizations; Be Tobacco Free; and Manage Stress. Content-based free applications for smartphones and tablets to complement the monthly healthy living messages; Lifestyle Evaluation Survey; Patient Centered Care information; Health Screening information; Designated daily place holders to measure Blood Pressure, Blood Sugar and Weight; Placement for monthly Goals; and VA Whole Health Model. Task 2- Provide VA Whole Health Journey eGuide   Provide a eGuides from a collection of resources (see table 1), in an eReader app format for up to 10,000 Veterans. The contractor should embed the code into the planner at varying locations for quick access. Table 1. Salient Characteristics/Design Specifications: The Calendar shall include new VA Branding throughout calendar (blue, red, white); The Calendar shall include VA specific imagery, logos and programs; The monthly Calendar shall be on one page; The Calendar shall have Large font minimum 12 pt font; The Calendar shall be a 28-page calendar plus an additional 4 page cover; The finished Calendar shall be approximate size of 8.5 x 16 (open) with an additional tab of 1.5 to 2 at the bottom of the back cover; The finished Calendar shall have color process, both sides, in and out, bleed; The Calendar shall be cover Gloss coated 80lbs, 8pts; The Calendar shall be saddle stitched; The Calendar shall have a hole to hang the calendar from. 6. Vendor Requirements: The contractor shall have previously worked with VACO to create and produce specific VA content; The contractor shall have experience in working with several programs across multiple networks; The Calendar content shall be derived from previously published and Copyright material that was specifically produced for VA Networks and Facilities; 7. Date(s) and place(s) of delivery and acceptance and FOB point: Delivery must be within 30 days ARO FOB: Destination Deliver to: Department of Veterans Affairs Cleveland VA Medical Center 10701 East Blvd. Cleveland, OH 44106 8. Applicable Provisions and Clauses: This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94,2005-95. At minimum, the offered item must meet all product specifications/salient characteristics of the requested item and be compatible with using the requested items disposables to be considered a responsive offer. 9. Offer Evaluation and Award. This is a competitive Request for Quotations (RFQ) that will be evaluated under FAR Part 12 and 13. The Government will issue a Firm-Fixed Price Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the requirements within this solicitation and will be the most advantageous to the Government, best value factors will be considered. The following factors will be used to evaluate offers: Technical (capability of offer to meet the Government s needs) Delivery/Installation Availability Price An Offeror s initial quote should contain the best terms from a price and technical standpoint.
