- Attachment (1), A00002, Q&R, dtd 9/15/17 Amendment A00002 The Contractor shall provide quality temporary Lodging for fifty four (54) crew members of USCGC SENECA from Period of Performance of 26 September 2017 - 4 November 2017, within a five (5) mile radius of 63 Flushing Avenue, Brooklyn, New York 11205. The contractor shall ensure that the lodging facility must have parking available for minimum of three (3) vehicles during the entire duration of the stay. The Contractor shall provide healthy, appetizing and nutritious dining, to include 3 meals a day and wait staff gratuities for up to fifty four (54) crew members of the USCGC SENECA, for period of performance of 26 September 2017 - 4 November 2017, starting with Lunch on 26 September 2017 and ending with Breakfast on 4 November 2017, all within a three (3) mile radius of the crew's primary lodging location and within a reasonable distance (not more than five (5) miles) of the shipyard location: 63 Flushing Avenue, Brooklyn, New York 11205 for Lunch. AMENDMENT A00001, The purpose of this amendment is to incorporate Contractor Evaluation Form, attachment (4) which was inadvertently not included with solicitation and to Respond to Questions, Attachment (5), received on 9/11/2017. This amendment A00001, does not change the quote submission date. Quotes are due on Friday, September 15, 2017 at 2:30pm ET.
