- Attachment J-6 - Bid Bond Form Attachment J-5 - State of Idaho Certificate of Verification of Workers’ Compensation Insurance Attachment J-4 - Past Performance Questionnaire Attachment J-3 - Project Plans Attachment J-2 - Project Specifications Attachment J-1 - Wage Determinations Request for Proposal - AG-02RC-S-17-0163 The US Forest Service anticipates awarding a firm-fixed price construction contract for the demolition and replacement of the Taxiway pavement at the USFS Air Tanker Base. The soliciation for this project will be competed using full and open competition and will be open to both large and small business concerns. This project was previously solicited under Request for Proposal AG-0343-S-17-0029 with no award being issued. Physical location of the construction will be 11569 N. Airport Drive, Hayden, ID 83835. Contractor shall furnish all labor, materials, equipment, supplies (other than for those listed as Government-furnished items) and supervision necessary to complete all required construction. The work includes but is not limited to removing and disposing of existing concrete and asphalt, excavation and embankment, compaction, furnishing and placing aggregate base courses, hot mix asphalt concrete, cast-in-place concrete and all other incidental items necessary to complete the project in accordance with the plans and specifications. The North American Industry Classification System Code (NAICS) is 238990 commercial and institutional building construction and the small business size standard is $15 million. There is no set-aside for this acquisition and it is issued using full and open competition. Per FAR 36.204, the estimated magnitude of construction for this project is between $500,000 and $1,000,000.
