Description

This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NAICS 339112, Standard Size 1000 Employees. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. Scope of Work Central Arkansas Veterans Healthcare System (CAVHS), ENT Clinic needs three (3) ENT Exam Chairs - S MAXI 4000, standard pillow articulating headrest, standard foam, standard armrest, special color in Gun Metal or equal stated in the Functional Statement Specifications. The vendor must also provide a service and maintenance agreement which will include adjusting, repairs, and replacing any malfunctioning equipment. Response to this notice should include a capability statement, and three (3) customer references to which you have provided similar services. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, and business size pursuant to the NAICS Code 339112, Standard Size 1000 Employees. Responses to this notice shall be e-mailed to Jacqueline.Simpson3@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than Friday, August 11, 2017 at 12pm CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Functional Statement ENT Exam Chairs BACKGROUND: Central Arkansas Veterans Healthcare System (CAVHS) ENT Clinic needs three (3) ENT Exam Chairs - S MAXI 4000, standard pillow articulating headrest, standard foam, standard armrest, special color in Gun Metal or equal that will meet the following functions: Maxi 4000 ENT Exam Chair Fully powered electro-mechanical lift and recline Lift capacity 400 lbs. (tested to 1,600 lbs.) 1 memory position with automatic return 345 °degree rotation with automatic lock function 17" travel range - 21" to 38" Child-Safety disable switch Multiple position articulating headrest Articulating footrest locks into up position for easy ingress/egress Lay flat armrests: Articulating armrests that go flat when reclined and allow for easy ingress/egress Wide 22" contoured seat to accommodate patients comfortably EVALUATION FACTORS FOR AWARD: The LPTA evaluation process will be accomplished as follows: Technical Acceptability. Each offeror s technical quotations will be evaluated to determine if the offeror provides a sound, compliant approach that meets the MEC required by the Government. The objective of the technical acceptability factor is to focus in on the go/no go assessment. Price Quotations: The offeror's Price quotations will be evaluated by CLIN Level and the offerors with the lowest overall Price will be ranked accordingly. CONTRACT TYPE: Firm-Fixed Price (FFP)
