- Medical Record Abstraction Application for Medical Monitoring Program The Centers for Disease Control and Prevention (CDC), Atlanta, GA, seeks interested sources to procure an electronic Medical Record Abstraction (MRA) application for its Medical Monitoring Project (MMP). MMP is a supplemental surveillance program designed to produce local and nationally representative data regarding people living with HIV/AIDS. The Government intends to procure this application as a Small Business set-aside. The following statement of work describes the requirement. The applicable NAICS Code is 541511, Custom Computer Programming Services. DISCLAIMER This sources sought is for informational purposes only and is not a request for proposals. It does not constitute a solicitation and shall not be construed as a commitment by the Government. Responses will not be considered offers and the Government is under no obligation to award a contract as a result of this announcement. No funds are available to pay response preparation costs incurred. Any information submitted by respondents is strictly voluntary. Responses, in three (3) or fewer pages, shall include: 1. Business name and address; 2. Name of company representative, business title and email; 3. Large or Small Business (include type) Size; 4. Cage Code; 5. DUNS Number; 6. Available contract vehicles to the Government for the procurement of the product and service; e.g., General Service Administration (GSA), GSA MOBIS, NIH, Federal Supply Schedules (FSS), or any other Government Agency contract vehicle. (This information is for market research only and does not preclude your company from responding to this notice if a vehicle is unavailable.) 7. Any comments/concerns regarding to the Draft Statement of Work (SOW) and/or NAICS code. 8. A brief capabilities statement demonstrating ability to perform the services listed in the attached Draft SOW to include the following: a. A brief description of experience in providing requirements similar to those described in the attached Draft SOW. b. If a small business, provide a brief statement of how your organization can provide a minimum of 50 percent of the work. Responses must demonstrate the company's ability to perform in accordance with the Limitations on Subcontracting clause (FAR 52.219-14). c. The capability package must be clear, concise, and complete. The CDC is under no obligation to provide feeback or to contact respondents for clarification of any information submitted in response to this request. Capability packages shall be limited to three (3) pages. Proprietary information and trade secrets, if any, must be clearly marked on all materials. All information received that is marked Proprietary will be handled accordingly. Please be advised that all submissions become Government property and will not be returned. All government personnel reviewing sources sought responses will have signed non-disclosure agreements and understand their responsibility for proper use and protection from unauthorized disclosure of proprietary information as described 41 USC 423. The Government shall not be held liable for any damages incurred if proprietary information is not properly identified.
