Description Statement and Purpose: The PHCCS was awarded the Year 2 DVPI grant to participant in the DVPI Purpose Area #1 project. The DVPI Project will be utilizing a Contractor for Trauma Informed Care & Women's Empowerment Project. The purpose is for the Contractor to provide training on how to become a sustainable individual. The participants will be learning new skills such as sewing projects, weaving projects, felting projects and other new arts & crafts. All of these will be done in accordance of the Trauma Informed care & Women's Empowerment Initiative in conjunction with DVPI services which is provide by PHC-CS. Location: Pinon Health Center Services Area and the seven surrounding Dzil Yiin Jijiin (Black Mesa) Region Chapter Houses. II. List of Services to be provided Contractor: a.) Contractor must be physically and be in person at the site of the program/session as a facilitator. b.) Contractor shall meet with PHCCs DVPI staff and partners to plan and coordinate program/session projects. c.) Contractor will conduct a Women's Wellness & Women's Trauma Care Empowerment; at one of the seven Dzil Yijiin (Black Mesa) Community Chapter House or Senior Center. d.) Contractor shall implement programs that accomplishes the PHCCS DVPI goals and objectives. e.) Contractor shall have the ability to work closely with PHCCS DVPI team to ensure programs are age and subject appropriate. f.) The programs/sessions will be about Domestic Violence and Bullying prevention with an emphasis in Trauma Informed Care regarding physical and sexual violence's by care takers. g.) Contractor will be introducing various learning skills in the capacity of Sewing fabric, bags, etc. and any new sewing techniques that individual can utilize towards their future endeavors. h.) Contractor shall conduct all-inclusive programs with sufficient supplies for each activity. i.) Contractor shall understand and ensure that the material and supplies are the property of PHCCS DVPI program and any remaining material and supplies are to be returned to the program j.) Contractor shall be willing to participate in focus group for reporting of the DVPI program. III. Period of Performance: July 01, 2017 thru September 30, 2017 IV.
