- Attachment 3, PP Questionnaire Attachment 2, PPI List Attachment 1, PWS RFQ, SF1449 Policy Letter The contractor shall provide three (3) Call Center Medical Appointment Line Clerks at the 374th Medical Group, Yokota AB, Japan, also referred to as the "Medical Treatment Facility" (MTF) herein under the subject contract. The contractor shall provide all labor, materials, tools, equipment, transportation, and supervision necessary to recruit, qualify, place, manage and retain clerks. Please refer to the attached PWS for the details. Note: SOFA/Logistic support will not be provided to clerks/other workers under the subject contract because SOFA is only applicable to a high-qualified or talented person(s) who cannot be replaced with any other person(s). Clerks/other workers under the subject contract do not fall in that category.
