Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 & 2005-95. This solicitation is set-aside for Service Disabled Veteran Owned Small Business. Must be verified and visible in www.vip.vetbiz.gov at the time of quote submission AND award. NAICS: 339113. All items must be new. Contract Line Items: ITEM DESCRIPTION QTY. 0001 Stainless steel scope cabinet with glass doors 18 D x 48 W x 92 H 4 EA 0002 Stainless steel scope cabinet with glass doors 18 D x 36 W x 92 H 3 EA 0003 Installation 1 EA The cabinets will need to hold about 40 scopes; total between all seven cabinets Will hold multiple types of scopes: gastrointestinal scopes, colonoscopes, EGD scopes; not an all-inclusive list. Endoscopes shall hang freely and not touch the ground or sides of the cabinets Padded backing is not required since the scopes shall not touch any sides Cabinets shall have a HEPA fan Cabinets shall have a drip tray Delivery (FOB Destination) Department of Veterans Affairs Salt Lake VA Medical Center 500 Foothill Drive Salt Lake City, UT 84148 The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical equipment only for VA Medical Centers.    No remanufactures or gray market items will be acceptable. (b)  Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.    All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions The provision at 52.212-2, Evaluation Commercial Items, does not apply to this acquisition. Evaluation will be made by LPTA. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with the offer. The clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. Any additional contract requirements or terms and conditions: FAR 52.203-99, FAR 52.232-40, FAR 52.247-34, VAAR 852.203-70, VAAR 852.211-73, VAAR 852.232-72, VAAR 852.246-70, VAAR 852.246-71. All Contractors must be registered in SAM.gov to do business with the federal government and annually ensure the information is current at https://www.sam.gov/.
