Attachment 02 - Contract Administration Plan Attachment 01 - List of Receiving Facilities N62645-17-R-0054 - combination synopsis/solicitation for commercial items This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a request for Proposals (RFP). Provisions and clauses in effect through Federal Acquisition Circular 2005-87 are incorporated. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses https://acquisition.gov/far/index.html and http://farsite.hill.af.mil/. The NAICS code is 339115, with a small business size of 1000 employees. The Naval Medical Logistics Command (NMLC) has a requirement for sixty-nine (69) computerized color vision tests (CCVTs) for Naval aviation clinics and Medical Treatment Facility ophthalmology and optometry departments and twenty-one (21) optional CCVTs for aircraft carriers and amphibious flat-decks. All qualified vendors are encouraged to submit a proposal. The solicitation number is N62645-17-R-0054.
