Description
Please be advised. This solicitation is being extended until 23 Jun 17, 4:00PM (CST). Please be advised. This solicitation has been modified. See attached Amendment 1 for further details. All offerors must sign Block 15A of the solicitation amendment and include this document with their proposal package. A document containing responses to contractor questions has been attached to this solicitation. (i) This is a combined synopsis/solicitation for commercial items prepared In Accordance With (IAW) the format in Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number FA8101-17-T-0014 is being issued as a Request For Quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) THIS ACQUISITION IS A 100% SMALL BUSINESS SET-ASIDE OR NON-PROFIT ORGANIZATION SET-ASIDE. The North American Industry Classification System (NAICS) code is 813110 with a size standard not to exceed $7.5 million (U.S.). (v) The following table lists the CLINs applicable to this requirement. NOTE: All CLINs are Firm Fixed Price (FFP). CLIN # Description Quantity Unit of Measure 0001 (Base Period) Protestant Religious Education Coordinator 12 Months 1001 (Option Year) Protestant Religious Education Coordinator 12 Months 2001 (Option Year) Protestant Religious Education Coordinator 12 Months 3001 (Option Year) Protestant Religious Education Coordinator 12 Months 4001 (Option Year) Protestant Religious Education Coordinator 12 Months (vi) The Air Force Sustainment Center Operational Division Services Branch (AFSC/PZIOA) located at Tinker Air Force Base (TAFB), Midwest City, Oklahoma, intends to award a FFP services contract for a Protestant Religious Education (RE) Coordinator for the Chapel. The contractor shall furnish all labor, materials, transportation, and incidentals necessary to serve as a resource person, advisor, and coordinator within the TAFB Chapel community. The contractor shall provide Protestant RE for all regularly scheduled Protestant worship services, as well as special services identified in the Chapel Annual Ministry Plan. Reference Attachment 1 "Performance Work Statement (PWS) Version 1" for further information regarding the services required. (vii) The place of performance is TAFB, OK 73145. The Period of Performance (PoP) will be 30 Sep 17 - 28 Feb 23. The PoP includes a one year base period, four option years, and an optional six month extension of services (to be used at the Government's discretion). Should the Government determine to use the optional six month extension of services clause, it shall be priced the same as the option year four CLIN price. (viii) The provision 52.212-1 "Instructions to Offerors-Commercial Items", applies to this acquisition. In addition to the requirements for the submission of offeror provided under paragraph (b) of the provision, the offeror shall also submit the following in response to the RFQ: • Evidence of one year of training or experience in RE and program management to include experience in recruiting volunteers and event planning. Reference PWS para 1.3.1. • Furnish a resume including verifiable academic, work related references, and two years’ experience working in a RE position in a Military Chapel setting. The contractor must provide evidence of appropriate professional competence by means of academic certification (a college degree and certification to teach is preferable). Documented evidence of the following is acceptable: two years’ experience coordinating and/or teaching religious education. Reference PWS para 1.3.2. (ix) Award will be conducted and evaluated under the provisions of FAR Part 12 "Acquisition of Commercial Items." IAW FAR Clause 52.212-3 "Offeror Representations and Certifications - Commercial Items: The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (r) of this provision." By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract resulting from this solicitation. Lack of registration in SAM shall render contractor ineligible for award. The Government advises Offerors to complete Offeror Representations and Certifications online at www.sam.gov prior to submitting an offer. (x) There is no Defense Priorities and Allocations System (DPAS) rating applicable for this procurement. (xi) All questions regarding this procurement are required to be submitted to the contract specialist on the attached spreadsheet via email NLT Monday, 5 June 2017, 12:00 p.m. (CST) Questions received after this date/time may not be considered by the Government. Proposals are due by Friday, 16 June 2017, 4:00 p.m. (CST). Proposals received after that time may not be considered. The response to this solicitation may be accomplished by electronic methods only. Contractor is required to: (1) Sign page 2 of the "Combined Synopsis & Solicitation" document; (2) handwrite and/or type the offered dollar ($) amount next to CLINs 0001, 1001, 2001, 3001, & 4001; and (3) email all required documents to the POC listed below. Proposals shall be valid for a minimum of 120 calendar days after proposal submission due date. NOTE: The combined synopsis/solicitation has two pages, and the actual solicitation begins on page 3.
