In preparation for a future procurement, the Michael E. DeBakey VA Medical Center in Houston, TX, is performing market research to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned, small business, or large business) relative to NAICS 339113 that can offer the services listed below: STATEMENT OF WORK Transaction Number: 580-17-3-167-0505 1. EQUIPMENT IDENTIFICATION: ENTRY # MANUFACTURER MODEL SERIAL # LOCATION 1 Trimano Support Arm Background: The Michael E. DeBakey VA Medical Center (MEDVAMC) is requesting procurement of Trimano Support Arm to be used for orthopedic procedures with the new sports attending surgeon. Project Scope: The overall scope of the project is: Item # Description/Part Number* Qty. AR-1640 Trimano Support Arm 1 AR-1641 Trimano Adapter 2 AR-1644 Trimano Beach Chair Kit 6 Freight 1 Supply Specifications: Salient Characteristics: Compatible with existing SKYTRON tables. Securely and safely holds the patient s arm in any desired position during arthroscopic or open shoulder surgery performed in the beach chair position. Sterile drape for the support arm Ergonomically-designed foam arm holder and COBAN. Warranty and Support: The warranty and support must cover all components of the requirement in accordance with standard manufacturer s warranty. Delivery Location: Michael E. DeBakey VA Medical Center 2002 Holcombe Boulevard Houston, TX 77030 The contractor shall not commence performance of the services identified in this SOW until the Contracting Officer (CO) has conducted a post-award conference, or has advised the contractor that a post-award conference has been waived. Interested companies should submit an electronic statement/attachment outlining their company's capability and capacity to provide the services requested. Also, firms should provide their business size status in accordance with the small business administration website (www.sba.gov) for the above referenced NAICS code they fall within (i.e. small businesses, 8(a) firms, HUB Zones, small disabled veteran owned businesses, etc.) This Sources sought is for informational purposes only and all businesses able to provide this equipment are encouraged to submit information regardless of Business size.
