- Attachment 3 Pricing Breakdown Attachment 2 Performance Work Statement (PWS) Attachment 1 Provisions and Clauses This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation W91364-17-T-0021 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation is set-aside 100 percent for Small Business. The NAICS code that applies is 721110 and business size is $32,500,000.00. This action will result in a FFP purchase order, utilizing simplified acquisition procedures under Full and Open competition After Exclusion of Sources. Requirement is for a 15 July 2017 Yellow Ribbon Event and lodging services in accordance with attached performance work statement. The Period of Performance (PoP) is for one day, 15 July 2017. Site location must be able to provide the following requirements and be within distance from Mansfield Lahm Airport Regional Airport, 2000 Harrington Memorial Road, Mansfield, Ohio 44903 of 65 miles or 1 ½ hours driving distance one way. Google or Bing maps will be used to verify distance by miles or time. Contract Line Items: this requirement and associated contract line item number (CLIN) is provided below: CLIN Description: Provide all personnel, supervision, and any items and services necessary to provide catered meals, and meeting rooms to the Ohio Air National Guard Yellow Ribbon Program, for a Pre-Deployment Yellow Ribbon Event scheduled for 15 July 2017 as defined in accordance with attached Performance Work Statement Attachment 2) Evaluations factors will be Lowest Price, Technically Acceptable. Technical Evaluation. List of Attachments: Attachment 1 Provisions and Clauses that apply to this solicitation. Attachment 2 Performance Work Statement (PWS) Attachment 3 Pricing Breakdown All questions are due no later than 15 June 2017 (1:00 P.M. EST). If all answers to questions submitted within this timeframe are not provided at least two (2) business days prior to bid due date, bid due date will be extended accordingly. All answers to questions (including any additional documentation provided) are distributed on a non-attribution basis.
