Page 2 of 2 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-17-Q-0736 is issued as a request for quote (RFQ). The North American Industry Classification System (NAICS) Code 332321 Metal Window and Door Manufacturing and a Business Size Standard of 750 employees is applicable. This is unrestricted however, preference will be given for SDVOSB/VOSB and small business. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89. No telephone requests will be accepted. Quote is requested for the following: DESCRIPTION/SPECIFICATIONS/WORK STATEMENT SCOPE OF WORK: The Contractor shall fabricate and assemble window units to be delivered to Ann Arbor VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. This contract is for materials, labor to assemble, and delivery. These units will be installed by in house employees to facilitate moving PCU and short stay. This request needs to be considered special due to the relocation of short stay and PCU to accommodate the ICU renovation Windows are to be activar or like: 11 Activar 30 X 66 mini blind glass unit vision lite kit for 1 ¾ thick door 2 Activar 24 X 24 mini blind glass unit with HM welded frame 1 Activar 36 X 24 mini blind glass unit with HM welded frame 6 Activar 48 X 24 mini blind glass unit with HM welded frame CONFORMANCE STANDARDS: All materials provided under this contract must be performed in conformance with the Federal regulations and AAVA policies and requirements. PARTS: Standard warranty for the unit will commence once the unit is installed. REPORTING REQUIREMENTS: The Contractor shall be required to report to the Maintenance Supervisor in Building 32 at the Ann Arbor VA Medical Center to log in during normal working hours. Normal business hours are 8:00am to 4:30pm Monday-Friday. This check in is mandatory. SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as OSHA may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) to VA Financial Services Center through the Tungsten (OB10) electronic invoicing system. Full text of the following clauses and/or provisions can be viewed through Internet access at http://farsite.hill.af.mil. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items or confirm this has already been completed in their SAM registration. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34. Offerors shall submit their quotation by e-mail to Brooke.Hansen@va.gov. Offers are due no later than 4:00 pm. EST, 19 June 2017. NO LATE QUOTES WILL BE ACCEPTED OR CONSIDERED. Any question must be submitted in writing and will be addressed by the Contract Officer and requesting service as quickly as possible. The successful offeror must be registered with SAM to be awarded a contract. You may register or update your registration on-line at https://www.sam.gov.
