Description
INTRODUCTION The Veterans Health Administration (VHA), Program Contracting Activity Central (PCAC) anticipates soliciting quotations and subsequently awarding a Firm-Fixed-Price contract for various procedure room equipment for the Eastern Colorado Health Care System. Acquisition of these items is part of the initial outfitting and activation of the new VA Medical Center being constructed in Aurora, CO. DESCRIPTION A contract is needed for the Eastern Colorado Health Care System for various procedure room equipment. The contractor shall deliver all items. All items must meet all salient characteristics and the contractor shall furnish all personnel, supplies, and equipment required for delivery. 2.5 SALIENT CHARACTERISTICS 2.5.1. Electrosurgical Unit, Compact w/Smoke Evacuator w/cart (JSN M3175.A) Quantity: 4 Brand Name: Bovie, Model Aaron Bovie 1250-G (See Attachment 3 CW EQ064 SSJ Over $150K) Specifications/Salient Characteristics Shall be a compact electrosurgical with smoke evacuation integration Shall have the output characteristics to perform Cut, Blend, Coagulation, Fulguration, and Bipolar functions Approximate Dimensions: Smoke Evacuator: 6 H x 11 W x 15 D Electrosurgical Unit: 10 W x 6 H x 12 D Mobile stand: 24 W x 24 D x 31 H Power Requirements 120V, 60 Hz Accessories/Configuration Options (per unit): Disposable Electrodes (ES20, and ES01) Qty: (1) Box ea Nonsterile tube and reducer (786T, SERF) Mobile stand and tray (ESMS-C) Disposable Pencil (ESP1) (1) Box Ea Reusable Grounding Cord (A1252C) and 5 disposable grounding pad (ESRE) included Smoke evacuator (SE02)   2.5.2. Electrosurgical Unit, Dual Output, Cautery (JSN M3175.B) Quantity: 5 Brand Name: Covidien/Valleylab Model Force FX C (See Attachment 3 CW EQ064 SSJ Over $150K) Specifications/Salient Characteristics Shall have an instant response technology that recognizes changes in tissue and adjusts voltage and current accordingly to maintain appropriate power. Shall have a power efficiency rating (PER) of no less than 98 for accurate and consistent cut performance. Shall accurately deliver the selected power setting evenly through resistant tissues. Shall perform at lower power setting thus minimizing the risk of tissue damage and neuromuscular stimulation. Shall have a Contact Quality Monitoring System Shall have the ability to recall most recently used mode and power settings Shall have an adjustable activation tone volume Technical Specification: Shall have three internal microcontrollers with the capability to reduce system reaction time and increase the processing speed. Shall have a spray coagulation voltage of no more than 9000 volts peak-to-peak output for broad but superficial coagulation. Shall have three cut modes- Low Cut, Pure Cut, and Blend Shall have four coag modes Shall have three monopolar Coag - desiccate (low), fulgurate (medium), and spray Shall have support for simultaneous coagulation Shall have two CEM modes Monopolar Cut (low) and Monopolar Coag Shall have three bipolar modes Precise (low), Standard (medium), and Macro (Microbipolar) Audio volume shall meet the requirements for IEC 60601-2-2. Approximate Dimensions: 4 3/8 H x 18 D x 14 W Power Requirements 100-120V, 60 Hz Configuration Options: The configuration of the electrosurgical unit at a minimum shall contain on the front panel: Power switch, controls for setting the modes and output power; a button for recalling the power settings and modes that were used last; receptacles for connecting electrosurgical accessories, and indicator that alert one to the current settings and patient return electrode status. The configuration of the electrosurgical unit at a minimum shall contain on the rear panel: Volume control; three footswitch receptacles; power entry module; equipotential grounding lug; and three ports (serial port, RF activation port, and expansion port). The internal configuration at a minimum shall consist of control (microcontroller) board; display board; footswitch board; power supply/radio frequency (RF) board; low voltage power supply; fan; and heat sinks. 2.5.3. Electrosurgical Unit, Dual Output, Endoscopy GI (JSN M3175.C) Quantity: 2 Brand Name: ERBE Generator Model VIO 300D (See Attachment 3 CW EQ064 SSJ Over $150K) Specifications/Salient Characteristics Shall have a plug-and play digital instrument recognition technology that automatically configures the entire system to one s programmed procedural parameters. Shall configure individually to meet the requirements of different medical specialties, indicators, and procedures. Shall have Cutting and Coagulation functions with easy of toggling between modes for desired tissue effect. Shall have a standalone generator Shall have a full-color screen user interface. Technical Specifications: Power output: Maximum CUT output is 300 watts at 500 ohms Safety system is Neutral Electrode Safety System Frequency is 350 kHz Maximum Coagulation function output is up to 200 watts Main connection Power frequency is between 50/60 Hz Power input during stand-by is 40 Watts Power input during max RF output is 500 Watts / 920V System voltage is 100-120V Has a potential equalization connection Standards Classification acc. Cart, Electrosurgical Unit, Universal (with JSN M3175.B) Quantity: 4 Brand Name: Covidien/Valleylab UC8009 Universal Mounting Cart Specifications/Salient Characteristics Mobile Cart designed to hold the Electrosurgical Unit in Section 2.5.2 above. Shall be constructed of rust-resistant material with four (4) conductive casters (2 locking, 2 non-locking) Shall include at least one (1) shelf and cord management capability Approximate Dimensions: 20 W x 26 D x 35 H Accessories/Configuration Options: N/A DELIVERY DELIVERY Delivery Timeframe: Not early than 6/1/17 and not later than 7/1/17. Acquisition of the above items is part of the initial outfitting and activation of the new VA medical center being constructed in Aurora, CO. Since construction is still occurring, delivery dates are approximate and will be coordinated with the successful contractor after award (see 2.6.1.7 Coordination). The Contractor shall contact the Onsite POC to schedule a pre-delivery meeting to be conducted approximately 60 days prior to the initial award delivery date for verification of delivery dates. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. A government-requested delivery delay up to 90 days after the delivery date specified in the contract award shall be at no additional cost to the government. Delivery Location: Contractor shall deliver all equipment to the Eastern Colorado Health Care System, 1700 N. Wheeling Street, Aurora, CO 80045. Items on delivery vehicles not equipped with a Lift Gate will not be accepted. The delivery will have to be rescheduled and any delivery charges will be the responsibility of the Contractor. Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufacturer s name marked thereon. Deliveries shall be marked with the PO and contract number. Delivery will be coordinated through the Onsite POC. Delivery personnel must comply with all posted site safety requirements, to include the wearing of PPE. Minimum requirements are hard hat, over the ankle boots, reflective safety vest, eye protection and gloves. If the requirements in sections 2.7.1 and 2.7.2 are not adhered to the delivery will be rejected and be rescheduled at Contractors expense. INSPECTION AND ACCEPTANCE 3.1. The Contractor shall conduct a joint inspection with the Onsite POC upon completion of delivery. 3.2. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. 3.3. The Contractor shall conduct a joint inspection with the Onsite POC after addressing all deficiencies. 3.4. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. 3.5. Disputes shall be resolved by the Contracting Officer. OPERATION AND MAINTENANCE MANUALS Binders - Quantity (2) each for items identified in section 2.5. 4.2. Digital Copies - Quantity (1) each for items identified in section 2.5. 4.3. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to Onsite POC upon completion of delivery. 5. OPERATOR TRAINING 5.1. Contractor shall provide all training materials at time of delivery. 6. PROTECTION OF PROPERTY 6.1. Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. 6.2. The Contractor shall perform an inspection of the building(s) and grounds with the Onsite POC prior to commencing work. The Contractor shall be responsible for repairing or replacing any items, components, building(s), or grounds damaged due to negligence and/or actions taken by the Contractor or its employees. The source of all repairs beyond simple surface cleaning is the facility construction contractor (or appropriate subcontractor), so that building warranty is maintained. Concurrence from the VA Facilities Management POC and Onsite POC is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 6.3. The Contractor shall be responsible for securing the items identified in section 2.5 and its own work tools and equipment during delivery. 7. SECURITY REQUIREMENTS 7.1. Security background checks are not required. 8. WARRANTY 8.1. The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. 8.2. The warranties shall include all travel and shipping costs associated with any warranty repair.
