- CLINs, Delivery and acceptance, Terms and Conditions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award a Firm Fixed Priced Contract for a Storage cabinet with HEPA filter for flexible endoscopes, iRIScope model, brand name or equal for the Brian Allgood Army Community Hospital in Yongsan, Korea. This is a request for quotation (RFQ). This solicitation number is W81K02-17-T-0316. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The Standard Industrial Classification Code is 6515, size standard is 750 employees and the NAICS code of 339113. This procurement is unrestricted. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See W81K02-17-T-0316Att1. Provision at 52.212-2, Evaluation -- Commercial Items, and its addendum applies to this acquisition. W81K02-17-T-0316Att1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. W81K02-17-T-0316Att1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition W81K02-17-T-0316Att1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award a Firm Fixed Priced contract for a one-time purchase. Contract Requirements - The successful offeror shall be required to provide the Storage cabinet with HEPA filter for flexible endoscopes, iRIScope model, brand name or equal, with the minimum requirements given in the attachment; W81K02-17-T-0316Att1. Contractors shall quote their unit pricing based on the quantities identified in each Contract Line Item Number.
