Purpose and Objectives: The intent of this Sources Sought Notice is to identify any and all business sizes who offerors are capable of providing ICS Chartr 200 VNG/ENG Two Channel, ICS Impulse V 3.0 USB Frenzel, 3.0 Lateral VHit Module, GN Otometrics Air Cal, Air Caloric Irrigator, ICS Impulse V 3.0 Larp/Ralp Module, GN Otometrics Otocam 300. The delivery point is The Department of Veteran Affairs, VA Butler Healthcare Sytstem, Health Care Center (HCC), 353 North Duffy Road, Butler, PA 16001. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Business name, address, Point of Contact, Business size information. All information is to be submitted via e-mail at donna.cooper2@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 3:00 pm (EST) on Friday, June 16, 2017. No submissions will be accepted after this date and time. Questions can be submitted electronically to donna.cooper2@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. Delivery, Installation, Calibration and Training A.1 INTRODUCTION- This contract is for the delivery, installation, calibration and onsite training and education for 1 ( one) ICS Chartr 200 VNG/ENG Two Channel, 1 (one) ICS Impulse V 3.0 USB Frenzel, 1 ( one) 3.0 Lateral VHit Module, 1 (one) GN Otometrics Air Cal, Air Caloric Irrigator, 1 (one) ICS Impulse V 3.0 Larp/Ralp Module, 1 ( One) GN Otometrics Otocam 300 for VA Butler Healthcare located at 353 North Duffy Rd, Butler, PA 16001. A.2 BACKGROUND- The ICS Chartr 200 provides a comprehensive test battery for diagnosing a wide range of balance and dizziness disorders allowing you to care for your patient from diagnosis through treatment. Whether you choose Video nystagmography (VNG)or Electronystagmography(ENG), you will benefit from a complete, portable, easy-to-use system. Advantages of the ICS Chartr are ability to see the patient s eyes at all times, video recording of the eyes which assists with diagnosis, no drift in the tracings, reduction of disposable supply cost. The ICS AirCal integrates the irrigation function with Otometrics VNG/ENG solutions enabling you to stay close to the patient while accurately completing the caloric test battery. With ICS Impulse ® you can easily assess and treat patients with vestibular symptoms using the Monocular Video Frenzel. The head impulse test (HIT) provides quick, clear-cut side of lesion specific assessment of the vestibuloocular reflex response to stimuli in the high-frequency range, the natural range of head movements. It is the only test that can assess all six semicircular canals. A.3 SCOPE OF WORK- 1 ( one) ICS Chartr 200 VNG/ENG Two Channel, 1 (one) ICS Impulse V 3.0 USB Frenzel, 1 ( one) 3.0 Lateral VHit Module, 1 (one) GN Otometrics Air Cal, Air Caloric Irrigator, 1 (one) ICS Impulse V 3.0 Larp/Ralp Module, 1 ( One) GN Otometrics Otocam 300 will be purchased and delivered to VA Butler Healthcare located at 353 North Duffy Road Butler PA 16001. Delivery shall include onsite installation, unpacking and installing equipment, configuring software, initial calibration and loading network settings. Training and education shall include 1 day of training up to three (3) clinicians. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1. The vendor: B1.1. Will provide delivery, installation and training for 1 ( one) ICS Chartr 200 VNG/ENG Two Channel, 1 (one) ICS Impulse V 3.0 USB Frenzel, 1 ( one) 3.0 Lateral VHit Module, 1 (one) GN Otometrics Air Cal, Air Caloric Irrigator, 1 (one) ICS Impulse V 3.0 Larp/Ralp Module, 1 ( One) GN Otometrics Otocam 300 will be delivered to Butler Healthcare Center. All deliveries will be coordinated between the vendor and VA Butler Healthcare based on the VA Butler Health Care Center construction schedule. B.1.1.2 The 1 ( one) ICS Chartr 200 VNG/ENG Two Channel must have the following specifications ICS Chartr 200 Hardware box Lightbar with ranging Remote control USB cable Isolation Transformer Binocular, dual purpose (vision and vision denied) video goggles (VG-40) VNG/ENG software for data capture, analysis and reporting ENG starter kit Eye tracking of at least + _ 30 degrees horizontal and +- 30 degrees vertical Power of 120 volts Resolution of at least 0.1 degree typical (horizontal and vertical) B.2 The 1 (one) ICS Impulse V 3.0 USB Frenzel must have the following specifications 2 packs face cushions (20 each) 1 pack vision denied cups (50 each) OTOsuite Vestibular software Laptop computer High speed USB camera Built In calibration lasers B.3 The 3.0 Lateral VHit Module must have the following specifications Lateral Canal testing capability Assess all six semicircular canals High frequency stimuli Analysis viewed in 2D, Hex Plot or3D B.4 One GN Otometrics Air Cal, Air Caloric Irrigator must have the following specifications Digital temperature read out Internally mounted compressor 120 volts Activation trigger on delivery head or foot control Less than 1 minute time to temperature Audible Beep Over temperature safety cut off B.5 One ICS Impulse V3.0 Larp/Ralp Module, when purchased with base module (USB Frenzel ) must have the following specifications Proprietary algorithms Training curve guide B.6 One GN Otometrics Otocam 300 must have the following specifications NOAH compatable Powerful light source Plug and play installation Capture full color video and images Built in heater The vendor will take the following equipment on traded-in: Trade in ICS VNG system # 594 and NCA 200 Irrigator #2212 The traded-in items cannot occur until October 2017. STATEMENT OF WORK PART C GENERAL REQUIREMENTS C.1. The contractor shall adhere to the job site requirements listed below: C.1.1. All personnel to adhere to site safety requirements PPE at a minimum to include hard hats, safety glasses, high-visibility clothing, hard sole shoes. C.1.2. All personnel subject to a 30-minute site safety orientation conducted by General Contractor (GC). C.1.3. Vendor responsible for unloading, handling, unpacking; clean up to dumpster provided by GC C.1.4. Vendor to schedule deliveries through VA who would in turn schedule with GC. At the time of these deliveries, most if not all products will have to go through the loading dock, so a schedule will be arranged for dock use. C.1.5. VA to obtain and maintain current certificates of insurance for each vendor C.1.6. Vendor responsible for protecting product after installation C.1.7. Standard work hours are Monday Friday, 7:00 AM 3:30 PM C.1.8. Contractor shall provide proof of insurance to COR before any work starts STATEMENT OF WORK PART D SUPPORTING INFORMATION D.1. Place of Performance VA Butler Healthcare Medical Center and/or Health Care Center (hereafter referred to as, The VA, VAMC, or HCC ) D.2. Period of Performance Period covers installation and verification/testing of operations to ensure the equipment operate as marketed. D.3. Special Considerations D.3.1. Contractor Furnished Materials and Services D.3.1.1. Equipment to transport equipment (e.g., dollies, pallet jacks, etc.) D.3.1.2. Tools necessary to finalize installation of equipment (e.g., installation of casters, setup of shelving) D.3.2. Government Furnished Materials and Services D.3.2.1. Elevator access, power, and as optimal an operating environment as can be reasonably achieved. D.3.3. Qualifications of Key Personnel Each party will determine the level of skills and adequate training for personnel supplied. D.3.4. Contractor s Statement of Release - In consideration of the modification agreed to herein as complete equitable adjustment, the Contractor hereby releases the Government from any and all liability under this contract for further equitable adjustments attributable to this modification. D.3.5. VA Butler Healthcare will provide a secure area for operation of the purchased equipment..
