- Sole Source Justification and Approval Combined Synopsis and Solicitation The Regional Health Contracting Office - Atlantic, Fort Drum, NY Contracting Cell intends to solicit, negotiate, and award a Single-Award Firm Fixed Price Service contract for Ireland Army Community Hospital, Fort Knox, KY for services on (4) Shimadzu Radspeed Systems with Canon DR Panels, (1) one Radspeed Analog, (1) one Sonialvision 16", (1) one RadPRO Mobile 3 w 70C as listed in the Statement of Work. This is a Brand Name requirement. Proposals shall provide technical documentation that the services conforms to the requirements listed in the Statement of Work. Proposals shall conform to the requirements of Addendum to 52.212-1 and FAR 52.212-1 Instructions to Offerors. Award will be made in accordance with Addendum to FAR 52.212-2 and FAR 52.212-2 Evaluation - Commercial Items. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is being solicited as sole source. This solicitation number W91YTZ-17-R-0045 is a request for proposal (RFP). The ordering period will be a base year of 01 June 2017 through 31 May 2018 with three one year option periods. The NAICS Code is 811310. The small business size standard is $7,500,000. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (as of 19 Jan 17). Interested persons may identify their interest and capability to respond to the requirement and submit proposals. All proposals received after the date of publication of this synopsis will be considered by the Government. Questions concerning this acquisition may be addressed to tiffany.m.narbone.civ@mail.mil no later than 21 June 2017, 4:00PM Eastern. Submit proposals via email ONLY to Tiffany M. Narbone at tiffany.m.narbone.civ@mail.mil no later than 14 July 2017 @ 4:00 PM Eastern. No telephonic request will be honored. Clauses and provisions throughout this solicitation can be viewed by accessing website www.farsite.hill.af.mil and http://farsite.hill.af.mil/vffara.htm. All offerors are cautioned that if selected for award, they must be registered with the System for Award Management (SAM) program. The SAM can be accessed via the internet at https://www.sam.gov/portal/public/SAM/. Confirmation of SAM registration will be validated prior to awarding a contract. Vendors shall include a completed copy of FAR provision 52.212-3 Alt 1, Offeror Representations and Certifications with their offer. Offerors shall include a confirmation of receipt of any amendments to this solicitation. Failure to submit a confirmation could result in the offer being determined non- responsive.
