Description
The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Farmington, Missouri.. The location where services are to be provided should be a within twenty mile radius of the United States Post Office, located at 102 E Columbia Street, Farmington, Missouri 63640. Performance periods under this contract will be for a one-year base period to begin on October 01, 2017 with a one (1) one-year option renewal periods with service ending (if all options are renewed) on September 30, 2019. The services required for this contract include a Base Year, Option Year One, respectively. The Following services are required and may be sub-contracted to another licensed contractor. Base Year: Estimated Number of Inmates = 80 Substance Abuse Treatment Services: Substance Use Intake (one per authorize inmate) for 80 inmates = 80 intakes. Intake Assessments & Report (one per authorized inmate) for 80 inmates = 80 assessments Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 80 = 3840 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 80 = 5760 units Family Counseling (30 minutes = 1 unit) 36 units (per inmate) x 20 inmates = 720 units Mental Health Treatment Services: = 60 Inmates Mental Health Intake (one per inmate) for 60 inmates = 60 Intakes Mental Health Intake Assessment & Report (one per inmate) for 60 inmates = 60 units Psychiatric Evaluation & Report = 1 report per authorized inmate x 60 inmates = 60 reports Crisis Intervention = one intervention for 14 inmates = 14 units Mental Health Individual Counseling (30 minutes =1 unit) 48 units per inmate for 60 inmates = 2880 units Mental Health Group Counseling (30 minutes =1 unit) 36 units per inmate for 60 inmates = 2160 units Mental Health Medication Monitoring (6 visits per inmate) for 60 inmates = 360 monitoring visits Transportation (124Trips) for 140 inmates = 17360 Units Option Year One: Estimated Number of Inmates = 80 Substance Abuse Treatment Services: Substance Use Intake (one per authorize inmate) for 80 inmates = 80 intakes. The request for quote will be available on or after June 19, 2017 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about July 19, 2017. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The Bureau anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
