- This request for information/sources sought notice is for planning purposes ONLY. The results of this market research will assist in the re-evaluation of the acquisition strategy in accordance with recent VA Procurement Policy guidance. VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice and does not guarantee an RFQ will follow. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify small businesses, and specifically, Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Business (VOSB) sources that have the resources and capabilities to provide one (1) Brand Name or Equal Global Surgical Corporation Dental Microscope as defined below for the Altoona, PA VA Medical Center. Dental Microscope: 4-Step AXIS Microscope, Floorstand: Part #: M A 1004 / 4-Step A-Series microscope, AXIS system, angle arm and coupler Part #: M A 1022-10 / Inclinable binocular, 0-220 degrees, f:160, 10X eyepieces Part#: M A 1028-250 / 250mm objective lens, 20mm fine focus adjustment Part#: M A 730F /Floorstand, mobile, total locking casters Part #: M A 730-HA / Horizontal arm, spring arm, extension arm (use with all floor stands, floor mounts & low wall mount) Part #: M A 801-LED / LED lightsource for the A-Series microscope, green and amber (UV) filter Part #: M A 1017 / Carr binocular adapter, 45 degree binocular extender Interested & Capable Responses NCO 4 is seeking responses from small businesses, particularly service disabled veteran owned (SDVOSB) and veteran owned (VOSB), who are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide this product. This is in accordance with the Supreme Court decision known as The Rule of Two. Please respond with: Business name, business type, socio-economic status (e.g., SDVOSB or VOSB), and person of contact (including telephone number & email address).
