Description
STATEMENT OF WORK Window Washing Chillicothe VA Medical Center GENERAL Furnish all labor, materials, equipment and supervision necessary to satisfactorily clean exterior window glass, screens and entrance glass (including door glass, transoms and side windows on a one-time basis in the area specified.) Buildings to have all windows and doors cleaned: All buildings are considered 3 story unless otherwise noted, 210,24,26,27,30,31(4 story and to include the outside awning to the entrance),35 (4 story),3 (1 story), 18,1,9,7 ( 2 story), 212,252 ( 1 story),28,29 ( 2 story), 247 ( 1 story),60,21 (1 story), 8 and all connecting corridors ( 1 story). Hours of operation for this contract shall be 7:30am-4:00PM, Monday-Friday. B31 to include the cleaning of the outside awning at the Urgent Care entranceway with the size dimensions of 95 feet X 28 feet, and also includes Building 31 Ambulance entranceway awning with the size dimensions of 40 feet X 28 feet. All work to be performed at the Chillicothe VAMC, 17273 State Route 104, Chillicothe, Ohio 45601. Date required: June 1, 2017 DESCRIPTION OF WORK Contractor will submit a cleaning detergent with specs along with a work plan and schedule before starting work, this will be approved by the COR. Contractor will wash windows thoroughly outside as specified. Windows are to be left transparent and free from dirt, streaks, water spots, smudges, tar, gum paint and other substances. Window frames, sills and muntins as well as door frames and jambs shall be washed along with window glass. Contractor will remove all screens and brush clean. They shall be made free of dirt, tar, gum, paint or any other substances and reinstalled in their original location. Contractor shall remove all excess water, splashing and washing compounds from sills, sashes, window frames and screens. Contractor is responsible for any damage done to the grounds surrounding the buildings where they have used lifts or drove their vehicle while washing the windows. These areas will be leveled out and filled in as necessary with approved backfill, seeded and covered with straw. Contractor is responsible for repairing any cracked or broken concrete cause by their lifts or vehicles. All work areas will be kept clean and fenced off using portable chain link fence sections so patients and employees are safe when contractor is working. No tools or material will be left unattended on the work site at any time. Contractor may not block exits or work overhead above exits without coordinating the work first with the COR. All work will be subject to inspection by the government at all reasonable times. Contractor shall report to the Assistant Chief of Environmental Management Section or his designee daily before beginning work. Contractor or his designee and the Assistant Chief Environmental Management Section or his designee will inspect the area to be done that day. Purpose is to list damaged windows and or screens to eliminate contractor responsibility for damage. The contractor, however, will be required to clean the damaged items noted. Failure to comply with these requirements may result in the contractor having to pay for damages other than those caused by their employees. The last thirty minutes of each day, the same person as noted in section I, will again inspect the area and note the progress and determine acceptability of work accomplished. If the day s work is found to be acceptable, the contractor may proceed to the next area. If unacceptable work is found, the contractor must correct work satisfactorily and in a timely manner. Contractor shall not be permitted to begin in another area until corrective work has been inspected and accepted by the contracting officer s representative (COR). If any window or screen is lodged in such a manner that the contractor cannot free it, this will be reported immediately to the COR. COR will make arrangements to have window or screen dislodged. The contractor will clean the window. If the contractor cannot re-lock a window or properly replace a screen, it must be reported to the COR before the end of the day. Window and glass cleaning personnel shall observe the requirements of all federal, and state safety standards. No outside window and glass cleaning work shall be performed when weather conditions such as rain, high winds, or ice, etc., are such that they add to the hazards of the operation. Smoking is prohibited except in designated areas. Make certain that all safety equipment has been inspected and is functioning properly. Squeegees, brushes, and other equipment used for cleaning windows above the first floor level shall be attached to the window cleaners person or the window cleaning hoist either by a strong rope or chain. This is to prevent the articles from falling and injuring someone, or damaging property. The contractor shall, when operating any window-cleaning hoist, barricade the area directly below the window cleaning operations with portable chain link fence sections. Contractor shall wear professional clothing at all times. Contractor shall be aware of possible sensitive material on computer screens. Contractor shall be responsible for covering/ protecting sprinkler heads located on the underside of the outside awnings of the Building 31 entranceway while cleaning awnings. Use of Hoist/ Lift The hoist/ lift used by the contractor to clean any exterior glass will comply with OSHA and all applicable regulations. It is also thoroughly understood and agreed that: Contractors supervisor must possess a 30 hour OSHA card and a designated competent person. All operators of the hoist/lift must have lift operators training documented. Fall protection must be used at all times. The type and specific conditions of the hoist is suitable for use. Storage of the contractor s hoist/ lift and appurtenances will be allowed at designated areas only. The contractor accepts full responsibility for the operation and use of said hoist/ lifts from any and all injuries and liability resulting from the use of same, and shall provide adequate insurance to cover any such injuries or liability. Contractor operates said hoist and its appurtenances in a safe and workmanship manner. Every precaution shall be taken in the setting up, rigging, and operation of said hoist/lift and appurtenances to insure against injury to persons and/or damage to building.
