Description: Pricing Sheet Statement of Work Oklahoma National Guard Solicitation # W912L6-17-T-0011 1. GENERAL INFORMATION This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. This solicitation is issued as a request for proposals (RFP). The Oklahoma National Guard (OKNG), under solicitation W912L6-16-T-0011, is soliciting vendors for an upcoming Oklahoma Yellow Ribbon Reintegration Program Pre-Deployment Event to be held in the Tulsa/Broken Arrow/Sand Springs/Sapulpa/Jenks Oklahoma area on 6-7 May 2017. The applicable NAICS code is 721110 Hotels (except Casino Hotels) and Motels. T he SBA small business size standard is $32,500,000.00. The prime vendor must complete at least 51% of the work performed to be eligible for award. See attached Performance Work Statement for detailed requirements. The Solicitation issue date is 22 March 2017 and will close on 12 April 2017 at 10:00 AM CST. Proposals received after the established deadline shall not be considered for this particular acquisition. The deadline for questions will be on 29 March at 4:00 PM CST. All questions must be submitted by email to trey.a.marshall.mil@mail.mil. All Solicitation amendments must be acknowledged in accordance with FAR Clause 52.215-1, Instructions to Contractors-Competitive Acquisition. Failure to provide the information requested by this RFP and any applicable amendments may render the Offeror's proposal incomplete and ineligible for further consideration for award. This Solicitation does not commit the Government to pay costs incurred in preparation and submission of initial and subsequent proposals or for other costs incurred prior to award of a formal contract. The Government anticipates making an award without discussions. Each Offeror is required to have an active registration within the System for Award Management (SAM) at time of determination of award and must list NAICS 721110 on its SAM registration. 2. EVALUATION Award will be based on an all or none basis using lowest price technically acceptable (LPTA) evaluation criteria conforming to the minimum requirements and specifications stated in the Performance Work Statement. Technically Acceptable Requirements includes meeting the specifications identified in the Performance Work Statement. Any deviations to the performance work statement must be submitted and approved by the Oklahoma National Guard as part of the question and answer process before a proposal will be accepted. Deadline for questions is 29 March at 4:00 PM CST. Offers that exceed funds availability or do not meet minimum requirements and specifications will not be considered for award. 3. PROPOSAL PRICING Each proposal will be itemized on the coversheet of the RFP on a separate attachment using the template provided with the Performance Work Statement. 4. PROPOSAL CONTENT The vendor shall review all submittals to verify conditions and to ensure accuracy, completeness and compliance with the contract requirements and shall indicate his approval on each submittal prior to submission. All variations from contract requirements shall be described in writing and approved by the Oklahoma National Guard prior to the closing of the questions window on 29 March. Approvals by the Contracting Officer will not relieve the vendor from responsibility for errors, omissions or noncompliance with contract requirements except where variations have been identified by the vendor and approved by the Contracting Officer. 5. PROPOSAL SUBMISSION Proposals are due not later than (NLT) 10:00 AM. CST on 12 April 2017. Offeror shall submit one (1) original proposal in hard copy or electronically thorugh the means authorized below. All proposals shall be prepared or formatted for standard 8.5" x 11" paper and use a legible font, which shall not be less than size 10 font. Each page shall be numbered and shall include the solicitation number. Pages shall have not less than a.5" margin on all sides. Proposals will not exceed seventy-five (75) pages excluding the vendor's representations and certifications. (d) Comptroller General Examination of Record. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports. (c) WAWF access. To access WAWF, the Contractor shall-- (1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and (2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site. (d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the "Web Based Training" link on the WAWF home page at https://wawf.eb.mil/. (e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data Interchange, or File Transfer Protocol. (f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order: (1) Document type. The Contractor shall use the following document type(s). Invoice and Receiving Report (2 in 1) (Contracting Officer: Insert applicable document type(s). Note: If a "Combo" document type is identified but not supportable by the Contractor's business systems, an "Invoice" (stand-alone) and "Receiving Report" (stand-alone) document type may be used instead.) (2) Inspection/acceptance location. The Contractor shall select the following inspection/acceptance location(s) in WAWF, as specified by the contracting officer. The Contractor shall use the information in the Routing Data Table below only to fill in applicable fields in WAWF when creating payment requests and receiving reports in the system. (5) WAWF email notifications.
