- Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W81K00-17-T-0156 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 and Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20161222. This solicitation is for a Full Service Support, preventive maintenance and calibration on a government owned electro-magnetic (EM) image-guided surgery system for Bayne-Jones Army Community Hospital, Fort Polk, LA. The period of requirement is 23 April 2017 - 22 April 2018, plus four (4) 12 month option periods. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219, with a size standard of $20,500,000.00. This procurement is being conducted as a Full and Open Competition. All eligible businesses may submit an offer, which will be considered. Offers are due by 31 March, at 10:00 AM, Central Standard Time. The point of contact is Mr. Efren Ramirez at (210) 221-4820 or efren.b.ramirez.civ@mail.mil. Offers shall be submitted via e-mail. Questions shall be submitted via e-mail not later than 28 March 2017. No questions will be entertained after this date. Bidder must: * Must provide all software upgrades issued by the equipment manufacturer. All repair parts shall meet exact number specifications and be the most current revision level. All parts must be new or re-manufactured by the original manufacturer of the instruments. * Must provide all necessary diagnostic software. * Must provide all replacement parts. * Must provide technical support via telephone 24 hours per day, 7 days a week via telephone. * Must provide on-site response within 6 hours of notification (in the event the system is disabled).
