- The following is a Sources Sought/Request for Information notice. This posted notice is not a Pre-Solicitation or Solicitation document. The government is not currently soliciting bids, quotes or proposals at this time; however, reserves the right to do so at a later date. The Coatesville VA Medical Center (CVAMC) located at 1400 Blackhorse Hill Rd., Coatesville, PA 19320-2040 has a future requirement to lease a mail assembly machine. The mail assembly machine shall be used to fold and insert letters and Business Replay Mail ( BRM) envelopes historically equating to 100,000 mailings per annum. The lease agreement shall include a new mail assembly machine, full service maintenance, and preventative maintenance. Responses to this notice shall include: Company Name Address Point of contact Phone Number E-mail Address DUNS Number CAGE Code Tax ID Number Note: All interested vendors shall be required to be registered in SAM(System for Award Management) in order to do business with the Government as all invoicing takes place via EFT. Please refer to SAM.gov. on how to register. Please provide answers to all of the below questions regarding your firm s Socio-Economic status pursuant to North American Industrial Classification Code (NAICS) 532420 Office Machinery and Equipment Rental and Leasing. Is your business a small business under NAICS 532420? Is your firm a Veteran-Owned Small Business? Is your firm a Service-Disabled Veteran-Owned Small Business? Is you firm a certified HUB Zone firm?
